Ambulatory Pharmacist - 20 hrs and as needed coverage for Hospital/Ambulatory Pharmacist -
Job Requirements
Role as Ambulatory Care Pharmacist
Ethically and lawfully execute and complete Registered Pharmacist duties.
Through initiation and maintenance of applicable Collaborative Drug Therapy Management (CDTM) agreements, provide comprehensive medication management for patients who have diabetes, take anticoagulants, wish to quit smoking, or meet other conditions.
Monitor ongoing patient medication appropriateness, and offer appropriate recommendations for patient with conditions that are not covered by CDTM agreements.
Additional Professional Development
Maintain educational requirements for all required licensure and certification.
Attend professional conventions and in-services on current pharmacy topics.
Annually demonstrate competence in Department equipment operation.
Perform other duties assigned by the Pharmacy Director.
