Position: Director of Social Services
Salary: $56,000 - $68,000
Key Responsibilities:
Lead and oversee the Social Services Department to ensure exceptional service delivery.
Conduct comprehensive patient assessments, develop individualized care plans, and support families during their transition into our facility.
Develop and implement departmental policies, addressing operational issues promptly and effectively.
Regularly evaluate patient needs, maintain open communication with families, and involve them in social service program planning.
Offer tailored solutions for social and environmental challenges, including financial assistance and community resource referrals.
Collaborate with interdisciplinary teams to provide holistic care for residents.
Represent our facility during regulatory surveys and inspections, ensuring compliance with all standards.
Serve as a liaison among residents, families, and medical staff, advocating for residents' social, psychological, and emotional well-being.
Requirements:
Degree in Social Work
Minimum of 3 years of experience in a Skilled Nursing Facility.
