ORGANIZATIONAL OVERVIEW                          

East Bay Agency for Children’s (EBAC) mission is to improve the well-being of children, youth, and families by reducing the impact of trauma and social inequities. EBAC strives to reduce barriers that contribute to disparities in wellness for socio-economically disadvantaged and racially marginalized families and to create communities where all children and families have support systems to reach their full potential. 


EBAC was established in 1952 when a group of parents of children with autism wanted to give their children the same educational and recreational opportunities that contribute to the physical well-being and social growth of all children. From this was born the first day-treatment program west of the Mississippi and was the foundation of growth in meeting the needs of children and families living in the San Francisco Bay Area, and especially in Alameda County. 


Today, EBAC’s programs serve over 10,000 children, youth and families throughout Alameda County each year with the goal of giving each child and family the specific resources, skills, and support they need to help them reach their full potential. POSITION OVERVIEW

Lead Care Manager (LCM) provides direct case management services for youth and families that qualify for our Enhanced Care Manager(ECM) services. ECM is designed to provide children with community-based case coordination to ensure that youth and family needs are met. The purpose of the role is to ensure that social, physical and mental health needs are being addressed to meet the end goal of youth and families thriving in their environments. The LCM provides culturally competent, family-centered, strength-based approaches of treatment. Services rendered will be in the East Bay.


Position Reports To: Enhanced Care Manager


Setting: Elementary, Middle School, High School 


QUALIFICATIONS

  1. Bilingual Spanish Required.
  2. Bachelor’s Degree in a Human Services discipline (social work, sociology, psychology, education, child development, etc.) desirable but not required. 
  3. Previous experience in working with high school/transitional age youth or families either in schools, client’s homes, or community settings. 
  4. Experience with meeting facilitation.
  5. Experience in social services and non-profit organizations preferred.
  6. Experience with Case Management service. 
  7. Experience with providing culturally relevant services.
  8. Ability to work with a multidisciplinary team.
  9. Excellent interpersonal and communication skills. 
  10. Skills in engaging and supporting youth to build resiliency are highly desirable.
  11. Ability to provide regular written documentation of services. 
  12. Position will receive extensive training. 


RESPONSIBILITIES 

Provides trauma informed navigation services, behavioral coaching and crisis support.

  1. Provides case management, advocacy, linkage, and support for clients and families to meet families identified goals. 
  2. Works as equal partners with family, family supports, and formal system partners to facilitate a family-centered, strength-based, culturally sensitive, needs driven planning process for creating individualized services and supports for children, youth and their families.
  3. Provides translation support when needed for culturally responsive services.
  4. Records progress notes and other charting requirements as necessary in a timely manner. (Training will be provided.)
  5. Meets service delivery expectations.
  6. Participates in staff meetings; trainings; and treatment planning and review when appropriate and as required.
  7. Develops supportive relationships with caregivers. 
  8. Participates in overall program development as a member of the treatment team. 
  9. Drives to and from site to meet with youth and families in their home/community settings. 
  10. Other duties as assigned. 


WORKING CONDITIONS and JOB SETTING

  1. Work is in school settings, in client homes, and community-based settings.
  2. Primary work is in under-resourced settings, in neighborhoods impacted by poverty, community violence, crime, systemic racism and oppression. 
  3. Ability to drive, with appropriate driver’s license and insurance; or access to a car; work requires travel between sites to attend meetings. 
  4. Periodic lifting up to 25 pounds is required.
