$70,000.00/yr - $85,000.00/yr

Accepting New Graduates or newly licensed/registered Clinical Dietician! Premier 243-bed facility in Bronx. Our Center provides high-quality, state-of-the-art services, which includes, but not limited to, skilled nursing and rehabilitation services, respiratory/ ventilation care, Diabetes management, wound care, etc. We deliver care with compassion by people who treat each resident as a family member.
Responsibilities
Provide Comprehensive Nutrition Assessment to meet individual patient requirements
Develop individualized nutritional care plans to establish standards of care and improve residents’ quality of life
Communicate with the interdisciplinary team
Attend Collaborative Care Plan (CCP) meetings with interdisciplinary teams
Complete MDS 3.0 accurately and timely
Provide nutritional counseling and education for residents and families with nutritional concerns.
Requirements
Candidates must be a Registered Dietitian
High comfort level with enteral nutrition
At least one year of experience in a long-term care setting
Experience with MDS 3.0
Strong oral and written communication skills
Experienced with electronic charting
Familiarity with state survey process and compliance to nutrition care regulations
