Christian Care Fellowship Square-Nursing Center is seeking an experienced and dedicated Social Services Professional to join our team. In this position, your primary objective will be to empower our residents to fully utilize their abilities and foster effective coping strategies that enhance their daily life quality and experiences.
The Social Services Professional will provide Fellowship Square residents with access to social service resources and ensure ongoing communication with residents and their families to affirm that residents can live securely in Independent Living. This role entails conducting the Life Steps assessment for residents during their annual lease renewal and aiding in recognizing the social, psychological, and emotional strengths and needs of residents. Responsibilities include coordinating placements within the facility or at home, adhering to State and Federal regulations, and the policies of Christian Care Health Center.
This short-stay rehabilitation facility has a low census, and experience in activities is a plus.
Responsibilities:
Conduct interviews with family members and, when possible, residents to determine assessment and needs prior to admission.
Schedule admission to the facility based on patient needs and availability.
Develop social history summaries to study, identify and provide for the social, emotional, and psychological needs of the resident.
Aids in residents’ adjustment to their medical and social condition and to our facility.
Works with residents and/or families in the planning of future discharge alternatives.
Encourages family contact and support of the resident.
Provides intensive and/or continuing counseling to residents in the event of a crisis or need.
Other duties as assigned.
Qualifications:
Bachelor's degree in social work is preferred.
A minimum of two years' experience in a healthcare facility's Social Services department is required.
A minimum of two years' experience in Long Term Care is required.
Must be able to articulate clearly and precisely.
The ability to effectively manage stress caused by ill residents, numerous tasks, noise, and interruptions, and to work collaboratively as part of the healthcare team while preserving a pleasant demeanor is essential.
Empathy and patience are essential.
The ability to cultivate trust and maintain long-term relationships with clients.
Exceptional interpersonal and communication skills.
