The June issue of the American Journal of Nursing (AJN) sheds light on a critical topic: Pressure Injury Prevention in Adult Critically Ill Patients. Maintaining skin integrity is paramount, especially for those in intensive care settings where patients are vulnerable to pressure injuries due to prolonged immobility. The JBI Evidence Implementation Summary demonstrates the importance of proactive measures in preventing these injuries. Pressure injuries not only cause physical discomfort but also pose serious health risks, including infections and delayed recovery. By improving the delivery of care to align with best practice, healthcare providers can significantly reduce the incidence of pressure injuries among critically ill patients. There are three components of the hospice plan: 🩺 Medical care includes recognizing pain and other symptoms, administering medications, anticipating complications, and learning what side effects may be associated with medications and treatments. 🐾 Nursing care includes reviewing treatments and medications administered by the family, reviewing the patient’s activities of daily living, assessing the patient’s condition, administering medications and treatments, and ensuring that all measures are taken to maximize the patient’s comfort. 💕 Support for the family consists of actively listening to owners elaborate on their feelings, validating the family’s experiences and showing empathy; providing information and other resources; and facilitating coping and decision-making.

LUNG SOUNDS🫁 As a clinically active nurse I ALWAYS ALWAYS ALWAYS listen to lung sounds at least once during my initial assessment, and then sometimes multiple times a shift! Lung sounds can tell you a TON about your patients before you even open a chart, and unfortunately, I have worked with multiple providers and nurses who asses lung (and heart) sounds from the doorway 🥴 I always try and stress the importance of physical assessments to new nurses/students that I precept because if not performed thoroughly, or even skipped in some cases, can cause immense patient harm! Assessments provide us a baseline for our patients and if we all just copy the previous charting (as I’ve seen in the past) then this opens the doorway for medical malpractice claims 😵💫 Bottom line- ASSESS YOUR PATIENTS! #legalnurseconsultant #RN #lungsounds #physicalassessment #medicalmalpractice
