Significant harm can be caused by poor documentation. And I mean harm to individuals, not an organisation having audit or accreditation issues It’s tough to take a moment to record notes, when there are constant competing demands. The frontline care workforce is pushed to the brink, and want to focus on care, not paper work. Only documentation is part of providing person centered, high quality and safe care.
Been struggling to write this in the shortest simplest way possible . Most organisations are amazing in the work that they do . But lack documentation, always remember what has not been documented did not happen . Over the last few weeks since beginning of the year we have been attending clinical assessments and it’s mainly Commision seeking justification of …. Or investigating why certain events happen . We often find that there is lack of documentation to justify. Most people call us after they read the report and say that’s not how it happened….. As a clinician you do not write ✍🏽 what is not documented. Many Providers are under investigation at present and we have been asked to consult and ensuring participants have all documentation and care plans in place .Often it’s too late to back track because the beginning was not set . My philosophy is always get things right the first time . Each client must have a personalised care plan not generalised . Surprisingly most providers have all the paperwork for audit but never or dont know when to implement them . Documentation helps the participant’s journey and on going care , funding etc. Ensure staff understand the importance of what they are doing Roles, responsibilities and duty of care . What ever role you are in today , ensure that you fulfill your end of the job/role. Seek clarification Document, document document 📃 we are part of chain /process . If anything happens every one’s role is assessed individually to ensure there were no gaps . Commission is knocking hard on peoples doors keep your eyes 👀 open . Have a good weekend ☺️

Rapport: The Heart of a Successful Consultation ❤️ In the rush of a busy clinic, it’s easy to focus on ticking off boxes and gathering data. But real consultations aren’t just about facts—they’re about relationships. The quality of the connection you establish with your patient can make all the difference. Why does rapport matter? Building rapport helps patients feel comfortable, understood, and heard. These first few moments of connection set the tone for the entire consultation, leading to better outcomes and more meaningful interactions. Here are a few key principles I teach GP trainees about rapport: 🔑 Be Warm and Present: First impressions count. Show genuine interest, make eye contact, and listen attentively. Your patients will feel it. 🚫 Avoid the ‘Interrogation Trap’: Rapid-fire questions may get you facts quickly, but they put a wall between you and the patient. You’ll miss the bigger picture. ✅ Use a Conversational Tone: Reflect back what the patient says to create a sense of partnership. This makes the patient feel acknowledged and more likely to share openly. Example: Interrogation Style: “How often do the headaches occur? On a scale of 1-10?” Conversational Style: “So, stress seems to be making your headaches worse?” 💡 The difference is subtle, but powerful. The second approach builds trust, making patients feel like partners in their own care, rather than subjects of an examination. By creating a safe space for patients to open up, you tend to get better insights—and that’s what makes consultations effective. #SCApreparation #MRCGPSCA #SCAexam

You can be really, really good at the tasks of your job, but if it's not contributing to your organisation, are you really being professional? Being professional means performing your work-role within the context of your whole organisation. This was incredibly well expressed by Professor Sir Bruce Keogh, former National Medical Director of the UK's National Health Service (NHS). In a document titled "Understanding the New NHS", he wrote: "As a medical student and a junior doctor I personally gave little thought to how the NHS worked – I was busy learning or looking after patients, and I felt that 'management' was someone else's responsibility. "Over the years I began to appreciate that this perception was misguided. "If I really cared about how well patients were treated then I had a moral and professional responsibility to understand the system in which I practised." 'Management' might indeed be someone else's responsibility when we are in the earliest stages of our career. However, we still need to understand it, for two reasons. Firstly, one day management will be a big part of your job! Secondly, 'understanding management' is an example of the value of seeking to understand the roles and responsibilities of colleagues throughout our organisation. Understanding the pieces and understanding the whole are all context for us. This context and insight helps deepen our understanding of our job and its purpose, and helps us deliver greater value. It means that we can focus on our part, our contribution and the purpose of our work-role while ensuring that we deliver true, contextual value to the rest of the organisation, our colleagues and clients.
