As a physician, you must keep in mind, my friend, that don’t work in isolation... Medicine is not a solo act!! This is not a one-sailor job!! From the doorman greeting patients at the door, to the CEOs making strategic decisions, every single person plays a role in the system. So be respectful to that, and to those people. Treat everyone with dignity. Your colleagues, the nursing staff, the janitors, the administrators. The moment you forget this, you diminish your value as a physician and fall short as a colleague. I've seen examples of such doctors, and trust me, they are not the ones you want to look up to.

The job list that keeps me organised Now I know not everyone will benefit from this template, and some do just fine writing on a blank sheet of paper but I'm a headless chicken without it, especially on long days and on-call shifts. So for those of you who operate similarly, feel free to save this or send me a message and I will send you a pdf for better quality. Thanks to medical students and foundation doctors who have given their feedback, I have made some useful changes to the template. Examples of how I used it on an on-call shift: Do now - r/v pt x - BP 210/103 - K+ 7.2 - NEWS 5, increased O2 requirements Decide when - discharge letter - maintenance fluids - cannula for Abx - post-op analgesia r/v - prescribe regular meds - micro discussion Delegate (I only tend to do this if I am swamped. The nurses are very lovely but please don't take advantage. Ever been stuck waiting for care due to a shift change? I’ve experienced firsthand how nursing handoff inefficiencies during shift changes can lead to significant delays and communication lapses. During my recent treatment session, I waited over 45 minutes without any nurse attending to me because of a poor handoff during a shift change. They basically forgot about me since I arrived just at the moment the outgoing nurse was signing out. This delay extended my treatment time by over an hour. I was PO'ed. Communication lapses and information loss during handoffs are dangerous. The most dangerous time in a hospital is during shift change! In fact, communication issues are a root cause in over 80% of sentinel events. Structured handoff protocols have been shown to reduce medical errors and preventable adverse events significantly. CALLING ALL MEDICAL SECRETARIES I know you want to help patient's through their pathway to treatment? I know you want to reduce the amount of queries you get from managers, validators etc. Thank you for those who are already doing this, you are making a difference! This is what I am asking you to do? Keep a track and manage those patients who are sent for a diagnostic test. Why: - You type the letter that states what tests they are having - You type the letter with the results to the patients and GP - You will reduce the amount of validation required - You will have stopped the patient's RTT Pathway at the right time - You will stop patient's worrying about what is happening - You will improve the patient experience because they will not have been missed What do you need to do: - create an excel spreadsheet to keep a list - add each diagnostic test that the patient is going for - check on a weekly basis whether results have been received - make sure the next step following clinical review is actioned - add an administrative clock stop to the patient's pathway on the Trust's EPR/PAS If you did all this and all medical secretaries did this would you not feel better that your family member should they be referred into the hospital would not get lost in the system and be assured that someone was looking out for them. #18weeks #waiting #waitinglist #HealthcareAdministration #medicalsecretary #nhs #NHScareers #NHSJobs #training #TrainingCourse #SeniorMANAGER

The importance of That First Assessment: Lately, I've reviewed several medical mal practice cases where a common issue kept resurfacing: failure to properly assess the patient from the start. Think about it-if you don't assess and document your initial assessment, how will you track changes in the patient's condition? Was the change gradual, or did it happen suddenly? Were you able to notify the physician in a timely manner? Failing to assess and document that crucial first assessment can have significant consequences for patient care.
