The pathophysiology of Meleney's gangrene involves a complex mechanism of tissue necrosis driven by the interaction of bacteria and the host's immune response. The infection typically begins in the skin and subcutaneous tissues, where bacteria enter through a break in the skin, such as a surgical incision, wound, or ulcer. Once inside, the bacteria release toxins and enzymes that break down the surrounding tissues. The body’s immune response, which attempts to fight the infection, inadvertently contributes to tissue destruction by causing inflammation and swelling, further impairing blood flow to the affected area. As the infection progresses, inadequate blood supply leads to tissue ischemia, exacerbating the necrosis. The affected area typically becomes red, swollen, and increasingly painful, with the skin eventually turning dark or black as the tissue dies. Unlike other types of gangrene, where tissue necrosis can occur rapidly, Meleney's gangrene develops more gradually, allowing for a window of opportunity for diagnosis and intervention if recognized early. Complex chest and lung infections with bronchial fistula are life-threatening situations with a mortality rate of up to 20%. If medical treatment fails, these patients require aggressive procedures to heal. Transposition of the omentum is a valuable, nonstandard option in these complex cases with aggressive infection involving the pleural space, with or without a bronchial fistula, when medical treatment is unsuccessful. We present a 29-year-old female patient diagnosed with primary immunodeficiency and invasive fungal infection with involvement of the left upper lobe and mediastinal and vertebral bodies treated with a lobectomy and intrathoracic transposition of the omentum. Science asked me to highlight the role of the vascular endothelium in complications of infections such as COVID-19 in this commentary. The principles of how the endothelium behaves during sepsis of course applies broadly - even as SARSCoV2 recedes as a current preoccupation, the mechanisms will replay in the next pandemic for which we much remain prepared.
