#Calcinosis of chronic renal failure can manifest as periarticular nodules mimicking crystalline arthropathy such as tophaceous #gout or calcium #pyrophosphate disease, or soft tissue deposits resulting in pain, ischemia, ulceration, or arteriolopathy, commonly termed #calciphylaxis. It is characterized by dystrophic calcium phosphate crystal deposits related to high calcium phosphorus product (Ca×P) commonly >70 mg/dL, and risk factors include end-stage kidney disease, longer time on dialysis (hemodialysis or peritoneal), hyperparathyroidism, diabetes, obesity, glucocorticoid use, chronic inflammatory diseases, warfarin use, female sex, protein C/S deficiency, hypervitaminosis D, and aluminum intoxication. *From: Lin, J. Lipopolysaccharide (LPS) is the primary structural component of the outer membrane of most gram-negative bacteria, serving as a protective barrier. However, high levels of circulating cytokines can have damaging effects, leading to chronic systemic conditions. Type II diabetes, rheumatoid arthritis, Alzheimer’s disease, Parkinson’s disease, gastrointestinal disorders (including increased permeability), cardiovascular disease, metabolic disorders, autoimmunity, and other chronic inflammatory disorders have been linked to elevations of LPS. In the clinical environment, the endotoxin LPS remains challenging and difficult to manage. Hi everyone Hope all doing good. A very nice nice case has been diagnosed so i want to share with all of you. A 20 years old young individual presented to us with a complaints of sudden onset chest pain, pedal edema and black stool since last few days. The patient also had a chronic backache and bilateral lower limb pain since 4 months and diffuse abdominal pain and breathlessness since last 1 month. The patient was was on medication of Nifedipine since last 4 months as antihypertensive. THE AGE IS VERY YOUNG WHEN WE SAW HISTORY OF HYPERTENSION. CECT abdomen pelvis showed multiple lytic sclerotic foci in lumbar vertebrae, bilateral pelvis bone and femur. Hepatomegaly with fatty infiltration and cholelithiasis was also noted. Overall findings were suggestive of either Marrow infiltrative disease or Multiple Myeloma. CT chest also showed similar multiple sclerotic lesions in dorsolumbar vertebrae, bilateral ribs, clavicle, scapula and sternum along with fractures in multiple ribs. Bilateral mild pleural effusion along with pulmonary artery hypertension was also observed. The findings were suspicious of metastasis. Aspiration was performed by taking proper written informed consent and it turned out to be metastasis. It is very unusual when karyotyping features revealing 47,XY, 8,der(14)t(13;14)(q12;q32). NOTE THE MARKED AREA THE PICTURE IS LITTLE BLURRED DUE TO EXCESS 1.5 ML OF VINBLASTINE.
