Of all the problems we face in clinical medicine, few are more vexing than patient interactions that don’t go well. I’d say doctors spend as much – if not more – time talking to each other about "difficult" patients as they do about difficult diagnoses. What Autumn Fiester, a philosopher and bioethicist at the University of Pennsylvania Perelman School of Medicine has to say about the difficult patient completely changed my understanding of challenging clinical interactions. I spent years of practice thinking I got it, but Autumn helped me realize I'd been missing the point entirely. There is no question that primary care is the solution to our health care crisis. When I think about the challenges facing family physicians and our health care system, I know one solution is to enact primary care-centered policies. A few weeks ago, I shared seven key points with Don Crane during our discussion at the #primarycaretransformationsummit.   1) Primary care is a common good that brings value to people across their lifetimes. Primary care is not something one only needs when they reach 50. Primary care has value from birth to death. 2) Health starts with primary care. To truly achieve a health care system whereby all individuals can live their most healthy lives, we need to ensure that all people have access to primary care throughout their lives. 3) Primary care needs and deserves a greater investment. We do not need to spend more money in health care – 4+ trillion is enough – we need to spend it differently and more purposefully. 4) Remove all the noise. Health care is too complicated, and we put too many barriers between people and primary care. Let’s make primary care more accessible and eliminate the administrative burdens that disincentivize people from seeking primary care. 5) Relationships matter. We need to create a system that allows everyone to have a longitudinal relationship with primary care throughout their lives. 6) Technology is important but it doesn’t replace the human interactions that drive comprehensive primary care. Primary care should be tech enhanced, not tech only. 7) Grow the primary care physician workforce needed to meet our population need. We must be more focused on growing the primary care physician workforce.   We at the AAFP believe we can do better, and that we should do better. Primary care physicians that are part of large health systems lead to increased spending and specialist visits (medicaleconomics.com). OHHH what a surprise! Independent value based primary care is proven to reduce health care expense AND improve health outcomes. Key Findings: Surge in Specialist Visits: The research revealed that vertical relationships between PCPs and health systems were correlated with a significant increase in specialist visits per patient-year. Compared to a control group, patients under the care of PCPs in these relationships saw a 22.64% rise in specialist visits, indicating a higher likelihood of receiving specialized care within the associated health systems. Escalation in Medical Spending: Patients whose PCPs had established vertical relationships with large health systems experienced a notable uptick in total medical expenditures per patient-year. This translated to an increase of $356.67 per patient-year, representing a 6.26% surge in health care spending when compared to the control group Marathon Health #vbc #primaryhealthcare #valuebasedcare
