MRI reports for low back pain… Did you know there’s a lot of variability depending on where you go? (See super interesting, and slightly concerning, study below). I had an interesting case this week (video below) where a reporting error that had been missed by multiple health professionals led to uneccessary fear, and almost led to uneccesary urgent escalation of care. MRIs are great tools and we’ve seen huge advancements in imaging technology. But as with many things, they’re not without risks/errors, and inappropriate use/overuse can be harmful. This was an interesting case where a peculiar/concerning finding led to the calling of the Radiology company for clarification… and thankfully the finding was a transcription error! MRI reports are also inconsistent between radiology centres. A study by Herzog et al., (2017) published in ‘The Spine Journal’ sent a patient with back related leg pain to 10 different MRI centres within a 3 week period and found: 📊 49 different ‘findings’ ☝🏼 16 unique findings and only found in a singular report 🤯 0 findings consistent across ALL reports ❌ each radiologist averaging 12 errors We talk more about imaging in episode 14 of the Empowered Beyond Pain podcast. Including how inappropriate imaging increases the risk for surgery and increases healthcare costs, and how most imaging findings (disc bulges, protrusions, herniations) resorb within a few months (not strongly related to whether pain/function improves) and are commonly found in people without pain. Note: To reiterate: scans are important for a portion of people with pain/back pain. But they must be interpreted correctly in the right clinical context: “treat the patient not the scan” comes to mind. Getting a second opinion, clarifying things, and making sure your healthcare professional is up to date with the evidence can be a worthwhile consideration. #lowbackpain #spineimaging

What sparked your interest in otorhinolaryngology and why did you choose to work at Mayo Clinic? A. Otolaryngology deals with many of the functions that help us to interact with the world around us. We tend to take them for granted, but when a problem occurs, they greatly affect our quality of life and may be life-threatening. The opportunity to have close relationships with my patients and help them regain such important functions is what initially drew me to otolaryngology. I chose to work at Mayo Clinic simply because of the unique approach to the patient care. It is important to me that patient care is the primary concern, with everything else being distant considerations. Mayo Clinic exemplifies this ideal. Q. What problems are you trying to solve and how will your solutions impact humanity? A. My clinical practice and research focus on restoring voice, swallowing, and breathing. Currently, it can be very difficult to restore proper form and function. I investigate tissue engineering and transplantation solutions to create better reconstructive options to restore these vital functions for my patients. Q. What excites you about Discovery Oasis? A. Discovery Oasis is a wonderful opportunity to bring together collaborative teams and create an environment that can accelerate scientific and healthcare advancements. I have personally seen the benefits of collaborations that include widely diverse experiences and knowledge that come together to focus on a specific problem. This sort of environment creates unparalleled opportunities to develop and accelerate healthcare solutions. Q. What is the most rewarding part of your job? A. The most rewarding part of my job is being able to develop and implement solutions that help patients in ways that were not previously available. To have the privilege of sharing in the joy my patients experience when they see improvement is indescribable. Until #interventionalradiology is recognised as a distinct specialty in Australia, patients will continue to have restricted #access to pinhole treatments. We are all trying to negotiate the glass ceiling above us but there is a widespread issue of suboptimal allocation of resources and inherent government restrictions because everyone thinks we are just "reporting scans". GPs and patients don't even know who we are or what we can do. The solution is top-down: specialty recognition, just like in the USA, Canada, Portugal, Japan, and the list goes on... Let's see what #irad can do: - Life saving treatments to stop bleeding in hospital patients or after motor vehicle accidents - Life saving treatments to solve sepsis in unwell hospital patients and outpatients - Limb-saving treatments for blocked arteries in the leg - Pinhole treatments for heavy menstrual bleeding that avoid hysterectomy - Pinhole treatments for lower urinary symptoms in men that avoid large prostate operations - Pinhole treatments to cure some types of cancer - Pinhole treatments to give chemotherapy directly into the centre of a cancer ...all this through an invisible hole and with lower cost than many open surgical alternatives. And yet, on many occasions patients still haven't heard of us or been given an option to see an IR - because on paper and with the government we don't (yet) exist! To me, there is immense value for patients, hospitals, and the wider healthcare system. Wouldn't you want a high-value IR department in your hospital? Well done to The Royal Australian & New Zealand College of Radiologists (RANZCR), Hannah Ireland, Justin Whitley for the advocacy video and all medical practitioners should support the cause.
