What is Physical Medicine & Rehabilitation? Physical Medicine and Rehabilitation(PM&R) is a transformative medical specialty dedicated to helping restore function and quality of life during illness or after injury. Physiatrists or PM&R physicians are critical leaders in the rehabilitation team who manage care for patients when other healthcare providers are not seeing them. They are in constant communication with the patient's surgery and physical therapy team to ensure optimal patient outcomes. The pathologies Physiatrists most commonly see range vastly from neurological to musculoskeletal, and there are a variety of subspecialties, with pain medicine, sports medicine, and traumatic brain injury being the largest.

🚨 Clinical Insight Alert! 🚨 Observed +ve sweep Test As on video (please excuse the half on half off glove - this is certainly not the normal practise ) Test performed on a male patient in his 50s. This simple yet effective test revealed the presence of excess fluid in the knee joint. For sure, not to be used in isolation. For patients in this age group, a positive sweep test can indicate potential underlying conditions such as osteoarthritis, meniscal injuries, or even more serious issues like a joint infection or inflammatory arthritis. While the sweep test, ballottement test, and fluctuation test are valuable tools for assessing knee effusion in clinical practice, their reliability and validity can vary. Combining these tests with imaging when necessary can enhance diagnostic accuracy and guide appropriate treatment strategies. Clinicians should be aware of the limitations and ensure they are well-practiced in the techniques to maximize the effectiveness of these assessments. ❄️The reliability and validity of these tests can be influenced by several factors: 🧠Clinician Experience: More experienced clinicians tend to have higher reliability in performing these tests due to better palpation skills and familiarity with the test procedures. 👥Patient Factors: The presence of other knee pathologies, such as severe osteoarthritis, can affect the accuracy of these tests. Patient body habitus (e.g., obesity) can also influence test results. ❗️Amount of Effusion: These tests are generally more reliable for detecting moderate to large effusions. Small effusions may be missed, especially if the clinician is less experienced. ⚖️Comparison with Imaging: Ultrasound or MRI can provide a more definitive assessment of knee effusion and can be used to confirm clinical findings. Why do treatment outcomes for the same surgery differ between the US and the Netherlands? In the Netherlands, patients participate in mandatory support groups, leading to better recovery. In the US, patients often go home alone. This highlights how loneliness affects treatment outcomes. Healthcare professionals also face stress and economic losses due to the epidemic of psychosocial disconnection. In a recently published article, our collaborate team from New England Baptist Hospital, Brigham and Women's Hospital, and Beth Israel Deaconess Medical Center discuss the costs associated with nonoperative versus operative management of meniscal tears in the 2 years after diagnosis. In our study from the MarketScan databases that included included 29,924 patients, the mean cost of management per patient was $3835. Costs were lower in the Non-Surgical group ($1905 ) compared with the Early Surgical group ($6759), while the highest costs were observed in the Late Surgical group ($7649). In summary, nonoperative management had the lowest cost burden and should be recommended for patients with appropriate indications.
