The attached article shines a light on a reason for rising health care costs. As more and more medical practices are being acquired by larger medical systems; there is a growing trend toward vertical integration of services, resulting in more intra-system referrals at higher prices. It is not a bad idea to ask price ahead of time so that you can get a sense of price variations from both within a major hospital system, and by contrast similar practices still independent and not subordinate to the dictates of higher, more consolidated ownership. One of the best studied care management programs for high-risk patients - the Camden Coalition Care Model - has yielded some fascinating results. The studies have shown that intensive care coordination, while it did increase ambulatory visits both immediately and long-term after a hospital discharge, did not reduce hospital admissions or impact patient outcomes. Further investigation shows that care management that does not attend to social and nonclinical needs is insufficient for complex, high-needs patients. This is important to consider for healthcare systems driven to improve patient outcomes while reducing spend. Home-based care is becoming less siloed and is integrating more and more into the healthcare continuum. One growing trend that supports this observation is the growing connection between Primary Care Physicians (PCPs) and Home Health agencies. Physician employment is one of the biggest trends reshaping the healthcare landscape. The migration from private practice to employment is happening across all specialties, especially primary care where 74% of primary care physicians worked for a hospital, health system, or other corporate entity in 2022. Breaking that down further, more than 50% of PCPs were employed by a hospital or health system. My colleague Ran Strul identified a handful of ways health systems can update their physician compensation models without adding undue stress to their organizations. #OWHealth

What does that mean for Employers bottom line and the quality of care employees are receiving?  See the attached, but here are a few highlights. … Between these groups, the researchers found that vertical relationships were associated with a significant, 22.6% increase in specialist visits per patient year, though there were no significant changes in total ED visits or hospitalizations. Within the specific healthcare system, for primary care physicians newly aligned with a system, specialist visits per patient year were 29.4% greater while within-system ED visits and hospitalizations per patient year were 14.2% and 22.4% higher, respectively. Total medical expenditures rose 6.3%, or just over $350, per patient-year ($30 pmpm) following a primary care physician’s entry into a vertical relationship. There were no significant differences in the probability of admission to a “high-price” hospital, readmission or admission to a hospital with low readmission rates between the study groups.
