One of the best studied care management programs for high-risk patients - the Camden Coalition Care Model - has yielded some fascinating results. The studies have shown that intensive care coordination, while it did increase ambulatory visits both immediately and long-term after a hospital discharge, did not reduce hospital admissions or impact patient outcomes. Further investigation shows that care management that does not attend to social and nonclinical needs is insufficient for complex, high-needs patients. This is important to consider for healthcare systems driven to improve patient outcomes while reducing spend. 🏥 Widespread consolidation in healthcare is affecting physicians, according to a new report from the Physicians Advocacy Institute (PAI) and Avalere Health. The report analyzed updated data from IQVIA OneKey database which contains physician and practice location information on hospital/health system ownership. "Based on your experience…" I will always advice my patients that before meeting with your General Practitioner, specialists, or other healthcare providers, it will be helpful to research your chronic condition. There is a wealth of information available, especially online. During your consultation, focus on asking questions that are less likely to be answered through general information. These should be questions from their personal experiences. For instance, you might ask, “Based on your experience, what is the best way for me to manage my condition?” or “Based on your experience, have you seen patients with my condition who have improved?” While doctors in public hospitals may have limited time for detailed discussions, most practitioners are usually very willing to share insights from their own experiences. #Singapore #TraditionalChineseMedicine #TCM #TCMSG Photo by Natalie Kinnear on Unsplash
