As MTM has grown, some issues have appeared that have not been properly addressed by the profession and others. A CMR for a patient with 5 meds is far different from a more complex patient with 15-20 meds, 10 supplements and taking a cannabis product. We now have pharmacogenomics (PGx). These types of patients require far more time and clinical acumen than the 5 med patient. They are sicker and the number of MRPs (medication related problems) can increase almost exponentially. This is analogous to comparing a procedure of a few stitches to transplant surgery or more complex orthopedic surgery with regard to time and training needed. These differences in complexity must be recognized by the profession, those outside the profession and payers. Our pharmacotherapy and PGx services see the most complex of the complex patients. These patients need highly trained clinicians to review their meds and because there is inadequate payment for this professional time, few if any of these patients have access to the more complex clinical pharmacist services they need. We are fortunate this need is recognized at our tertiary care facility and supported for complex patients. However, in just about every setting these types of services operate at a financial loss if offered at all. Adequate supportive reimbursement must be received commensurate with the training and time!
