''I have little illusion in [mental health treatments], let alone the antidepressants drama. You have to take them for at least six weeks, often with all the side effects, only to conclude they don’t do anything..'' #Depression, affecting over 300 million globally, remains a significant and complicated challenge with up to 30% of patients finding current treatments, including #psychotherapy and #antidepressants not fully effective. A recent study explored the perspectives and experiences of 11 patients undergoing a double-blind randomised clinical trial with a single session of #psilocybin treatment for treatment-resistant depression, offering insights into the treatment's effectiveness and areas for improvement. The findings reveal that patients faced challenges with trust-building, managing expectations, and navigating the profound and often overwhelming experience. They expressed a general desire for a more comprehensive treatment approach, which may include multiple, dose-escalated, psilocybin sessions, and ongoing therapy to integrate and make sense of their experiences. Relax. No one’s in control. In this open-label study, a subset of 24 patients with treatment-resistant depression (TRD) received either 15mg of psilocybin or 100mcg LSD in combination with psychotherapy; subjective drug effects and depression scores were measured. Patients with TRD showed a 29% improvement in symptoms after psychedelic-assisted therapy (with psilocybin or LSD). Greater relaxation during psychedelic dosing sessions predicted greater improvements in depression symptoms. According to the authors, “Importantly, psilocybin and LSD were similarly effective in improving depressive symptoms. … The potential role of relaxation during psychedelic-assisted therapy deserves further discussion, as it emerged as a particularly strong predictor of antidepressant response.” (Link in comments)

VA Funds Studies on New Therapies for Treating Mental Health Conditions The VA has released a request for applications (RFA) inviting proposals from its network of VA researchers, in collaboration with academic institutions, to study the use of psychedelic compounds such as 3,4-methylenedioxymethamphetamine (MDMA) and psilocybin in treating posttraumatic stress disorder (PTSD) and depression. This marks the first time since the 1960s that the VA is funding research on these compounds. The initiative aims to gather definitive scientific evidence on the potential efficacy and safety of psychedelic compounds when used in conjunction with psychotherapy to treat veterans with PTSD and depression. The move is part of the VA's commitment to exploring innovative approaches to improve the health and quality of life for veterans, aligning with the Biden-Harris Administration's focus on veterans' well-being. Research on these compounds will be conducted under stringent safety protocols and regulatory approvals from the United States Food and Drug Administration (FDA) and the Drug Enforcement Administration (DEA).

Depression is common amongst older people residing in long‐term care facilities. Currently, most residents treated for depression are prescribed antidepressant medications...but are psychological therapies effective? 📚 🔍 New #SystematicReview looks at the evidence from 19 studies with 873 participants with #depression Key messages – Psychological therapies (sometimes known as talking therapies) may treat symptoms of depression better than non‐therapy approaches in older adults living in long‐term care (LTC) facilities (for example, nursing or residential homes and assisted‐living facilities). – Psychological therapies may also be better than non‐therapy approaches for increasing quality of life and psychological well‐being (an individual's emotional health and overall functioning) in the short term. 🚩 The evidence for these effects is very uncertain, limiting our confidence in the findings. Something not talked about - "hope trauma." When there is a lack of adequate preparation, results are over promised or under delivered, there is a lack of experience and perspective to contextualize disappointing results, or hope is manipulated, unintentionally or not - vulnerable and desperate populations are at risk of trauma from the hope of a substantial and meaningful outcome. Statistics are wonderful at helping us navigate complex decisions as to what types of therapies or interventions to seek, what results to expect, and what larger populations experience, but the reality for those individuals who fall outside of the statistics is a feeling that they've been lied to or are "too different" to experience what the majority do. This article is a great example of how "hope" must be tempered at times with nurturance, support, and rationality, so it is not falsely inflated due to excitement or individual biases. It's also important to not extinguish hope when the power of belief, and the power of the mind, can also play a vitally important role in ushering in change through the mind-body connection. It's important when discussing the potential benefits of any individual therapy to also make out potential alternatives and risks clearly and thoroughly so that expectations can be managed, and disappointments (should they occur) softened. But, sometimes, even with the most thorough preparation, people that are desperate, hopeful, and vulnerable for relief, the lack of response or positive benefit can be crushing. And, it's our job as healers to be able to help catch these clients and offer a soft landing, so they can learn to catch themselves. #mdma #psychedelicmedicine #hope #trauma
