No surprise here. If this country wants to get serious about training and maintaining a vital Primary Care workforce, it needs to value and compensate Primary Care visits significantly more. This is in addition to an overhaul of our current third-party and employer-based insurance model for financing health care and tort reforms needed to slow down the seemingly inexorable increases in health care costs in this country. The stresses of having to maintain a broad-base of medical knowledge amid rapid medical advances including a growing diversity of drugs, devices, and best practices statements, needed by the Primary Care Provider combined with lower compensation is a sure way to encourage further migration of future Physicians and Advanced Practice nurses and Physician Associates away from Primary Care and towards a narrower scope of practice and the more financially lucrative specialty care fields.

How can we address the growing gap between the healthcare needs of the aging population and the lack of providers? “Not only is the supply of healthcare providers decreasing, but demand is increasing. The Association of American Medical Colleges reports that by 2034, the number of Americans over 65 will grow by more than 42%. With age comes a greater likelihood of illness, and the need for primary care physicians and specialists is expected to spike.” There are 2 key ways we can address this. The first is logistics. We need to meet the aging population where they are at, including their homes, as many face barriers in getting to a doctor or using telehealth. The second is engaging nurses to provide more front line preventative care services in the field. There are many ways nurses can help fill the gap in the field. For example, conducting a medication reconciliation to identify medication non-adherence or interactions that could be contributing to health decline. Nurses can conduct fall risk or health risk assessments in the home to enable early interventions that could prevent a serious health event from occurring. Nurses can also work with patients to create a patient centered care plan helping the patient drive towards indivualized health goals to improve their health. They can also assess for social determinants of health that might create a barrier to care. Nurse practitioners can provide primary care in the home.
