Your 2024 Medicare Choices and How They Affect Your Taxes
As we step into 2024, it’s important for Medicare beneficiaries to understand how their coverage choices impact their federal taxes. While the enrollment decisions have been made, there are still important considerations for managing the tax obligations effectively throughout the year. Here’s what you need to know and tips for managing them.
Do I have to pay for Medicare Part A?
You usually don’t pay a monthly premium for Part A coverage if you or your spouse paid Medicare taxes while working for a certain amount of time. This is sometimes called premium-free Part A. If you aren’t eligible for premium-free Part A, you may be able to buy it.
If you buy Part A, you’ll pay a premium of either $278 or up to $506 each month in 2024 depending on how long you or your spouse worked and paid Medicare taxes.
If you need help paying your Part A premium there is help available. If you have limited income and resources, you may be able to get help from your state to pay your Medicare costs if you meet certain conditions.
In most cases, if you choose to buy Part A, you must also have Part B and pay monthly premiums for both. If you choose NOT to buy Part A, you can still buy Part B if you’re eligible

What is the Additional Medicare Tax?
As a result of the Affordable Care Act (ACA), the Additional Medicare Tax was added in 2013. Additional Medicare Tax rates are 0.9 percent and apply to highly compensated workers.
Individuals are liable for Additional Medicare Tax if their wages, compensation, or self-employment income (together with their spouse's income if filing a joint return) exceed the threshold amount for their filing status.
You may have to make estimated tax payments if you anticipate that you will be liable for Additional Medicare Tax but will not be able to satisfy the liability through Additional Medicare Tax withholding.
In 2013, the Additional Medicare Tax went into effect and applied to wages, compensation, and self-employment income over a threshold amount received in taxable years after December 31, 2012.
