Perioperative Spinal Cord Injury in high-risk adult deformity surgeries has relatively low incidence, but can have devastating consequences. Thus, appropriate postoperative care, follow up, and timely management are essential, and these procedures should only be performed by experienced spine surgeons with skills and expertise in deformity correction. NICHD established the Pelvic Floor Disorders Network (PFDN) in 2001 to encourage collaborative research and improve patient care on a common women’s health condition. Today, PFDN researchers published a new study in JAMA Surgery, where they directly compared three procedures that are performed to repair pelvic organ prolapse. From this randomized clinical trial, the study team found that all three surgeries are generally comparable and safe after three years. These findings provide healthcare providers and patients with clinical evidence to discuss the pros and cons of available surgeries and decide upon the most appropriate treatment. A total of nine studies were included, and the results showed that both surgical modalities had good safety profiles in the early and long-term stages. For early endpoint events, the risk of all-cause mortality and major bleeding within 30 years was similar in the staged TAVR + PCI and the contemporaneous TAVR + PCI (P > 0.05). Additionally, the risk of myocardial infarction, stroke, acute kidney injury and pacemaker implantation within 30 days or perioperatively is similar (P > 0.05). For long-term endpoint events, the risk of all-cause mortality, myocardial infarction and stroke was similar in the two groups at ≥2 years (P > 0.05). In patients undergoing TAVR who required coronary revascularization, no significant differences were observed in the early and long-term outcomes between those receiving concurrent TAVR and PCI versus staged surgery.

A study aimed to see how often patients on blood thinners get immediate or delayed brain bleeding after a mild head injury and whether they need short-term hospital stays based on initial scans and symptoms. This study's review of 696 patients found that immediate brain bleeding occurred in 5.1% of cases, with no link to the type of blood thinner used and delayed bleeding was very rare. Symptoms like loss of consciousness and headache were indicators of higher risk, suggesting that initial scans alone should not determine the need for hospital observation. A teenager with chest pain on exertion 16 y/o girl presented with chest pain during sport and decreased exercise tolerance. Cardiological work-up revealed myocardial bridging of the LAD over a length of 7 cm, with reversible ischemia on SPECT myocardial perfusion imaging. Given the morphology of intramyocardial course and severity of symptoms, surgical unroofing of the LAD was scheduled. Surgery was uneventful, with quick postoperative recovery. · Myocardial bridging is a common coronary anomaly, in which a coronary artery runs through heart muscle, needing multimodal imaging to find those needing treatment. · Beta-blockers and calcium-channel blockers are first treatments, with revascularization for those not responding to medication. · Revascularization options include PCI or surgical myotomy, chosen based on lesion specifics, atherosclerosis, expertise, and patient preference. https://lnkd.in/extx2xup
