A supportive work environment involves a workplace where employees have good relationships with one another, good communication, and there is clear and fair leadership on board. A strong correlation exists between a supportive work environment and job satisfaction among veterinary profesionals that can lead to increased work commitment and effort shown by employees at work. This leads to increased productivity, better physical and mental wellbeing for the employee, and less staff turnover for the employer. High work-related stress and low job satisfaction among veterinary professionals can lead to many adverse outcomes for individuals and the practice, such as higher rate of decreased work performance, and increased frequency of errors and accidents at work. A supportive work environment can act as a buffer against stresses and strains in an individual's personal and professional life because the camaraderie and concern for one another that occurs in a supportive work environment can lead to emotional, instrumental, and informational support for employees during hard times. This can be seen as an investment for both the employer and the employee, leading more dedication to the practice and more effecient employees, showing how important this is in preventing staff turnover and keeping employees satisfied in their current position. De nouvelles recommandations émergent sans cesse et elles mettent souvent en défaut ce que nous croyions savoir. L’un des domaines où ces recommandations évoluent rapidement , et parfois en profondeur, est la fluidothérapie. Après des années d’approche agressive, les nouvelles bonnes pratiques deviennent douces et mesurées. La surcharge volumique paraissait toujours moins risquée et préjudiciable à l’animal que l’hypoperfusion, l’hypovolémie, ou l’état de choc. Cet abus dans l’usage des fluides a possiblement tué autant de chats qu’il n’en a sauvé. Le mot d’ordre aujourd’hui est donc: CONTRÔLE et MESURE. L’American Animal Hospital Association remet les choses en place grâce à ses nouvelles lignes directrices 2024. La Société Francophone de Médecine Féline SFMF approuve ces guidelines et ajoute quelques points pratiques: 😸en situation peranesthésique, sur animal stable et contrôlé, sans hypotension ou risque d’hypotension, sans pertes et avec une durée d’anesthésie courte, la perfusion peut ne pas être nécessaire. 😸en cas de doute sur l’état d’hydratation , d’anesthésie profonde, de pertes visibles ou sensibles plus importantes, une recharge par bolus à effet de 1-2 ml/kg devient la règle 😸 sauf cas particuliers et exceptionnels les débits supérieurs à 10-20 ml/kg/h doivent être limités 😺la diurèse forcée avec des débits élevés (>30 ml/kg/h) a peu d’indications chez le chat et son efficacité n’est pas prouvée 😺chez les chats âgés, potentiellement porteurs d’une cardiomyopathie, une échocardiographie (pré-anesthésique voire per-anesthesique), et/ou un test proBNP permettent d’affiner encore le protocole anesthésique et fluidique. 😸l’eau n’est rien sans l’air: une bonne perfusion permet une bonne oxygénation et vice versa. La fluidothérapie sera encore meilleure et efficace associée à une oxygénothérapie. 😺le débit n’est pas le seul paramètre à contrôler, il faut aussi tenir compte du volume total administré. Parfois, ce n’est pas un débit excessif qui entraîne la surcharge, mais le volume total qui dépasse les capacités de l’animal. 😸 le traitement par fluides ne se fait pas par des tables ou des calculs mais par des évaluations cliniques répétées.

“Unpredictable and uncontrollable” is a difficult way to describe the reasons why vets are leaving. I believe it’s an incorrect way to look at things that doesn’t factor in the millions of reasons vets have to leave. Veterinary as a whole is in a difficult spot - there just isn’t enough time & vets to help all the animals that need care. There are different solutions for time, Mlemo included. What needs a more robust and systemic solution is the amount of vets currently in practice. Maybe if we recognised the issue is partly the culture, partly how we handle educating vets (only 1 university in Ireland has a suitable course! a lot of future vets have to go to the UK or other EU countries as there just isn’t enough places in that course for everyone that wants to attend it), and all the other factors; we could try coming up with some other solution that fixes things longer term. I’m open to conversation about this, feel free to comment or DM your thoughts.
