When a patient receives a cancer diagnosis, their journey ahead is filled with the unknown. From treatments to countless appointments, every day is consumed by their diagnosis, and the added stress of financial uncertainty should not also weigh on their shoulders. Patient navigation programs effectively enhance equal access to care, diminish financial toxicity, and improve health and quality of life outcomes. However, resource constraints can hinder the widespread use of these programs. Are we all represented in cancer clinical trials? Unfortunately, not yet. An important recent study in JAMA Network Open reveals significant disparities in cancer clinical trial availability in the U.S.: “Social Determinants of Health and the Availability of Cancer Clinical Trials in the United States.” ▪ Socially vulnerable counties are significantly less likely to have cancer clinical trials available compared to less vulnerable counties. ▪ Alarmingly, the disparity in trial availability has worsened over the past 15 years, with the most vulnerable areas having only a fraction of the trials. ▪ The study underscores the need to expand clinical trial opportunities in socially vulnerable communities to ensure equitable cancer care. The Montefiore Cancer Center in The Bronx, NY studied the effectiveness of the MyEleanor app, an AI-based virtual patient navigator, as part of a colorectal cancer screening quality improvement (QI) project. The study explored how AI-driven applications, such as the MyEleanor app, could potentially serve as a complement to a stretched healthcare workforce. The findings of the study showed the potential for AI-driven healthcare applications to provide valuable support within the healthcare sector. The authors conclude that the integration of technology and healthcare, as evidenced by the promising results of the study, presents an opportunity to address critical healthcare needs and improve patient outcomes. The significance of the intersection of technology and healthcare is huge. I think that the study makes a good argument for an EB-2 NIW petition for a computer science professional working on merging technology and healthcare. Remote monitoring and data collection are pivotal for decentralized clinical trials (DCTs): - Enhancing Trial Accessibility and Participation: Remote monitoring and data collection eliminate geographical barriers, enabling more patients to participate in clinical trials, especially those who cannot easily access trial centers. This helps expand the sample size, improve data representativeness, and enhance the reliability of trial results. - Reducing Trial Costs and Time: Decentralized clinical trials typically reduce time and cost burdens for both researchers and patients, as patients do not need to frequently visit trial centers. This also reduces the need for physical trial facilities, thus lowering the overall cost of trials. - Improving Real-time and Accurate Data: Remote monitoring and data collection enable real-time collection of patient physiological parameters, symptoms, and treatment responses, providing researchers with more timely and accurate information. This helps in early detection of patient condition changes, thereby improving trial monitoring and management efficiency. - Protecting Patient Privacy and Data Security: Remote monitoring and data collection must strictly adhere to data privacy and security standards to protect patients' personal information and data security. Researchers need to take necessary measures to ensure the security and confidentiality of data to mitigate the risk of data breaches or misuse. - Facing Challenges and Limitations: Such as the requirements for technical devices, patient acceptance of digital technology, data collection consistency, and standardization. Additionally, for certain diseases or specific populations, remote monitoring may not fully replace traditional clinical observation and examination.
