Daily Critical Care Pearls #105: Ketamine and Propofol Combination 💉These two medications are frequently given together for procedural sedation in the Emergency Department and elsewhere. This combination is sometimes called “Ketofol”. 💉In this combination, propofol serves to provide sedation while ketamine serves to provide analgesia without increasing the respiratory depressant effects of propofol. 💉An additional benefit of the combination is that the hypotension from propofol may be partially offset by the positive cardiovascular effects of ketamine. 💉The ratio of ketamine to propofol varies widely with some studies using 1:1, some using 1:10, and several more at various points in between this range. #criticalcare #pharmacy #medicine #clinicalpharmacy #pearls

💉 Propofol is the most commonly used intravenous anesthetic that is currently used in various procedural sedations, including dental sedation. Its use is preferred owing to advantages of rapid onset of action, short duration of action, and easy titration. However, despite these advantages, it carries risks of respiratory depression and hemodynamic. 💉 Ketamine is an anesthetic preferred owing to its analgesic effect and the maintenance effect on airway reflex. Nevertheless, its clinical use is controversial because of disadvantages of dissociative anesthesia, postoperative hallucinations, sympathomimetic effects, and stimulations of saliva and secretions instability. ⚠️ Use of a combination of ketamine and propofol (ketofol) as a sedative has been attracting attention in various clinical fields. The combination of ketamine and propofol can reduce the dose of each drug and compensate for the disadvantages of the other through the antagonistic characteristics of both drugs, resulting in beneficial results for successful sedation. 🎯 Ketofol has been used in bolus form to provide sedation and analgesia for painful procedures in the emergency room. How to Reduce Alprazolam Safely! After long-term treatment, rapidly reducing the dose or stopping alprazolam abruptly can trigger severe withdrawal symptoms, which can be dangerous. To avoid these risks, it is crucial to taper off alprazolam gradually. Typically, the dose should be reduced by 0.5 mg every three days. For those on alprazolam for a long time, switching to a longer-acting medication like clonazepam or diazepam before tapering can help minimize withdrawal effects and ensure safety. #pharmacy #medicationsafety #patientcare #pharmd

Daily Critical Care Pearls #69: Management of Angioedema from ACE Inhibitors ✔️ Protect the airway with endotracheal intubation. Nothing else matters. ✔️ Antihistamines, glucocorticoids, and epinephrine don’t work. ✔️ Icatibant doesn’t affect clinically meaningful endpoints. ✔️ Ecallantide doesn’t work. ✔️ Fresh frozen plasma might help. ✔️ If you hear stridor, see that the patient is drooling, using accessory muscles to breathe, or has edema of the tongue or floor of the mouth, assume that the physician will want to immediately intubate the patient and obtain the necessary medications to assist with this process (Etomidate-Induction, Succinylcholine-paralysis) ✔️ Obtain appropriate sedation to use post intubation ✔️ Be prepared for hypotension to occur after intubation and have a liter of normal saline and a vasopressor readily accessible. #criticalcare #pharmacy #medicine #clinicalpharmacy #pearls

Daily Critical Care Pearls #77: Severe Alcohol Withdrawal - Part 2 - (Non-benzodiazepine treatments: Dexmedetomidine, phenobarbital, propofol) 📌 Dexmedetomidine: Blunts Tremor Tachycardia Anxiety Agitation, Provides sedation, Reduces benzodiazepine requirements, Does not protect against seizure risk! 📌 Phenobarbital: Very helpful in refractory DT when benzos fail, Risk of intubation rises greatly - be prepared for intubation if phenobarbital is added. 📌 Propofol: Intubation is required for use.
