In today's healthcare system, reflecting on lessons learned and educating the population I serve are key priorities. This weekend, I encountered a patient whose goals were neglected, physicians weren't listening, and his sister was advocating tirelessly for him. In Value-Based Care, discussing patients' goals at every visit is paramount. In hospice, we adapt care plans daily to align with patient and family goals. Recognizing the importance of timely hospice care within the broader value-based care model is essential. As healthcare professionals, we must prioritize integrating hospice care into value-based models. This ensures that patients receive compassionate care tailored to their needs and preferences.
