The best treatment is complete removal of bladder with prostate (in men) and uterus (in women), with removal of nodes and creation of new bladder or a channel (conduit) using small intestine. This procedure can be performed by open surgery as well as laparoscopic surgery with/without robotic assistance. The difference between open and minimally invasive is the extent of blood loss. This study investigates the monocyte-to-lymphocyte multiplying platelets ratio (MLPR) as a novel inflammatory marker for predicting acute kidney injury (AKI) after cardiac surgery with cardiopulmonary bypass (CPB). The study presented is a systematic review and meta-analysis evaluating the long-term oncologic outcomes of patients undergoing duodenum-preserving pancreatic head resection (DPPHR) as opposed to the standard pancreatoduodenectomy (PD) for benign, cystic, and neuroendocrine pancreatic tumors. The analysis included 38 cohort studies, totaling 1,262 patients. Key findings include a high concordance rate (over 90%) between preoperative diagnoses and final histopathology, suggesting accurate selection of patients for DPPHR. The procedure demonstrated a lower in-hospital mortality rate and reoperation frequency compared to PD, although the difference was not statistically significant. Importantly, there was no significant difference in local tumor recurrence rates between the two methods. The study concludes that DPPHR offers a viable, less invasive alternative to PD with the potential for similar oncologic outcomes and fewer surgery-related complications, indicating a promising step forward in pancreatic surgery.

#InterventionalRadiology (IR: Interventional Renaissance) + #OrthopaedicOncology = #MinimallyInvasive Impactful Patient Care: A case of renal cell cancer osteolytic metastasis to the left acetabulum (with other oligometastsatses). Current Orthopaedic Oncological Procedure: Currenrly and traditionally, embolization is done 0-2 days before the procedure in the Interventional Radiology suite and then total hip arthroplasty using 3D mega implants are done in the Operating Room under second anesthesia. Postoperatively, patients stay in the hospital for 1-2 weeks and may experience many complications. #Percutaneous #MinimallyInvasive #InterventionalOncology care: For this particular case, Dr. Igor Latich (Interventional Radiologist at Yale) and I teamed up for a minimally invasive procedure. Concurrent Angiography through the right femoral artery and Left Acetabulum #AORIF (#Ablation-Osteoplasty-Reinforcement-Internal Fixation) was done in the same room under one anesthesia. #AORIF is an ambulatory #MinimallyInvasive procedure that addresses cancer, biomechanics, and bone biology. Renal cell cancer skeletal metastasis is known to be hyper-vascular and bleeding profusely during the open acetabular surgeries. SANITAS part of BUPA- Madrid Hospitals. Vascular and Endovascular Surgery and Interventional Radiology Team. We present 3 cases of carotid body tumor treated in our Hospital last month. Case 1: 46-year-old woman, history of high blood pressure and dyslipidemia. Diagnosed with Shamblin II carotid body tumor. Percutaneous embolization with onyx and resection 2 days later without complications. Case 2: 63-year-old woman, history of breast cancer. Right cervical lymph node dissection due to suspected malignancy a year ago. Diagnosed with Shamblin II carotid body tumor. Percutaneous embolization with onyx and resection a week later. Intervention without complications. Case 3: 75-year-old man with a history of presyncope. Diagnosed with Shamblin I carotid body tumor with possible irritation of the vagus nerve as the cause of his symptoms. Percutaneous embolization with onyx and resection one week later. Intervention without complications. All cases were embolized by the Neuroradiology Team (thanks Dr. Caniego) In our experience, prior embolization with onyx is the key to avoiding bleeding and making a safe resection. It is not essential to perform the resection immediately. This is one of the best graphs I have seen about the impact of Prehabilitation for people with cancer awaiting surgery. Evidence shows prehabilitation reduces length of stay, post-operative pain, and post-operative complications. However currently, the promotion of physical activity is not part of routine cancer care. At One Wirral CIC we have been delivering a Cancer Prehabilitation service for 2 years supporting over 1,500 cancer patients prior to surgery or treatment. So far we have seen some great outcomes such as 5% reduction in Length of Stay, Reductions in readmissions for colorectal and urological cancer patients reducing from 9.9% to 7.9%, 98% reported increased or maintained physical activity, 93% reported reduced or maintained levels of fatigue and 99% maintained their clinical frailty score. #Prehabilitation #cancercare #personalisedcare #optimisation
