In the social policy classes I teach, we often discuss the role of nonprofits and how they can both help and inadvertently hold complex problems in place. Providing respite nursing home housing for unhoused older adults with serious medical conditions is essential. Still, it may also prevent policymakers from addressing the core issue of lack of affordable housing. Balancing immediate needs with sustainable change is crucial. What do you think? 👏👏👏 for Urban Pathways Total Wellness Program in helping our consumers live healthier lives AND reducing visits to the ER and 911 calls! A few things to highlight: 💡 The work is not done when someone has a roof over their heads and this is just one of the many ways Urban Pathways helps our consumers on their path to independent living. 💡 Nonprofits can be a powerful source of innovation filling an experimentation gap that the government and for-profit sector aren't ready to address but more often than not, the funding for these new projects comes from private donations so... 💡 ...If you give, please consider using your dollars to fund not just day-to-day work but also new (even unproven) programs so we can validate new solutions for systemic change. Ans if you're looking for some inspiration, take 3 minutes to read this Gothamist article on the program and the people impacted. Thank you Karen Yi for shining a light on our work! https://lnkd.in/gMzZWWCn #endhomelessness #totalwellness #innovation

⚙️🔨Building a System of Care for a Healthier Southern Nevada Homelessness and poor health are often intertwined. Recognizing this, the City of Las Vegas is leading the design of a responsive system of care focused on improving healthcare access for people experiencing homelessness. This week, our team facilitated the second design session with city and non-profit leaders. We focused on building the framework for a system where people experiencing homelessness in Southern Nevada have the resources they need to live healthy, hopeful lives. Health is more than doctor visits; food security, transportation, and stable housing are crucial for well-being. A "no wrong door" approach ensures people can access the services they need, regardless of their situation. This person-centered model is key to improving health outcomes for those facing disparities. What are your thoughts on this approach? #SocialDriversofHealth #CommunityHealth #PersoncenteredCare

April is #NationalMinorityHealthMonth. This month forces us to recognize the deep health disparities in our city and country. Racial and ethnic minority populations face higher rates of poor health and disease than their white counterparts. What can an organization like BCS do to combat this issue? The first step is addressing the social determinants of health (SODH), like education, poverty, racism, housing, and other non-medical factors that affect community health. By identifying and working to improve issues like these, we are one step closer to a more equitable Brooklyn. Reflecting on the findings of the Abbotsford Substance Use System Access Journey Mapping Project, it's clear that we need to act now. I've seen too many people—loved ones included—unable to access the services they need to survive. Frontline workers shared heartbreaking stories of policy related barriers, delays, a toxic drug supply, and a lack of treatment options. The project, commissioned by the City of Abbotsford and conducted by UBC's Centre for Advancing Health Outcomes, involved interviews, focus groups, and field observations with frontline workers from 25 different agencies. Their experiences revealed severe shortages in treatment options, long wait times, and the devastating impact of toxic drugs. This is an urgent issue, not just in Abbotsford, but across the province. We need to bridge these gaps and give people the support they deserve. Let’s fight for real change. Utilizing funds from a SparcBC Homelessness Community Action Grant, the City of Abbotsford partnered with UBC’s Centre for Advancing Health Outcomes to conduct a collaborative research initiative that would identify and address barriers that exist for unhoused individuals in Abbotsford in accessing substance use services. The research compiled insights from frontline social service workers representing 25 different agencies, involving 27 interviews, focus groups, and field observations that focused on the experiences of both the service providers and clients. The report highlights key challenges, including the lack of treatment options, toxic drug supply, and systemic delays that hinder timely access to detox and other services. These findings support the City of Abbotsford and other stakeholders in advocating for improvements to services and funding, ensuring that those in need can access critical services without delay. This project reflects Abbotsford’s ongoing commitment to addressing the impacts of substance use, homelessness, and well-being in our community. The issue of homelessness has escalated due to the pandemic, with over 653,000 people reported as homeless in 2023. Healthcare organizations, encumbered by frequent visits from homeless individuals to emergency departments, are unable to tackle this challenge alone; collaboration with other community organizations is essential. Nonprofit organization Community Solutions aims to eliminate homelessness by fostering relationships between healthcare systems and homeless response systems. They work towards measurable reductions in homelessness, lower healthcare costs, and improved general population health. Their pilot initiative, Healthcare and Homelessness, focuses on data sharing, identifying patients in need of housing services, preventing discharge into homelessness, and improving coordination between healthcare and housing services.
