50 year old with left carotid occlusion and severe right carotid stenosis with a small stroke treated with carotid stenting via radial access. We have stopped doing pre plasty when using the wallstents. Saves time and steps and limits complications. One of those “keep it simple” cases where we dont even have to enter the aortic arch. Cricothyrotomy Master Class Note: Initial breaths after securing the airway should be delivered via manual bag compressions and lateral chest squeeze after each breath delivered (rather than with a ventilator) while maximal medical therapy is continued. Once switched onto the ventilator, use 6mL/kg volume, low I:E ratio, RR 8-12, no PEEP initially, and avoid breath stacking. The risk of obstructive hyperinflation and pneumothorax If the airway maneuvers and medicines are not effective immediately, consider pneumothorax and do not hesitate to perform bilateral finger thoracostomies if necessary. #emergencymedicine #cricothrotomy #surgicalairway

Using the F.A.S.T. test involves asking these simple questions: 🟠 Face – Check their face. Has their mouth drooped? 🟠 Arms – Can they lift both arms? 🟠 Speech – Is their speech slurred? Do they understand you? 🟠Time – Time is critical! If you see any of these signs, call triple zero (000) immediately.

The Pelvic Binder… An unresponsive or decreased LOC patient with multi-systems trauma from an MVC, auto-ped, significant fall, or other mechanism, who is hypotensive, tachycardic, pale, and no obvious external bleeding…… My opinion, apply the pelvic binder, along with blood products, TXA, and rapid transport (of course airway if needed and resuscitate before you intubate). Assessing pelvic pain on a patient that has a decreased level of consciousness can be unreliable and performing a physical exam on an unresponsive pts pelvis can disrupt formed clots. The short amount of time this binder would be in place should not cause significant to tissue damage, however, the benefit in the presence of an open book pelvic fracture, could be life versus death.
