The Pelvic Binder…
An unresponsive or decreased LOC patient with multi-systems trauma from an MVC, auto-ped, significant fall, or other mechanism, who is hypotensive, tachycardic, pale, and no obvious external bleeding……
My opinion, apply the pelvic binder, along with blood products, TXA, and rapid transport (of course airway if needed and resuscitate before you intubate). Assessing pelvic pain on a patient that has a decreased level of consciousness can be unreliable and performing a physical exam on an unresponsive pts pelvis can disrupt formed clots. The short amount of time this binder would be in place should not cause significant to tissue damage, however, the benefit in the presence of an open book pelvic fracture, could be life versus death.
