As a practicing ENDOCRINOLOGIST, I am happy that GOOD NEWS came today!!! In that, we provided the FIRST HAND EVIDENCE to the world, of an otherwise NEGLECTED SCENARIO. The evidence of which, has just been published at PLOS ONE journal some few hours ago. TAKE HOME MESSAGE: Hypoglycaemia (Low blood sugar level) in newborns is just as prevalent as normoglycaemia!!! Moreover, the FIGHT between paediatricians and endocrinologists, on whether or not to TREAT 'ASYMPTOMATIC neonatal hypoglycaemia', is still being worked upon by the same research group. What's already known about the TOPIC: 1. Hypoglycaemia is the COMMONEST metabolic adaptation among newborns. 2. Persistent neonatal hypoglycaemia is associated with a number of myriads - mental retardation as well as 'lifelong' neurological deficit (e.g. recurrent seizures) What's new from our work worth the world to know: 1. Hypoglycaemia can also exist in 'ASYMPTOMATIC phase' (about 50% in our study population!) among newborns, especially those born before their age (PRETERM newborns) 2. 'Breastfeeding within 1st hour after birth' stands as the STRONGEST FACTOR in prevention of ASYMPTOMATIC hypoglycaemia even in PRETERM newborns. What drove Kelvin to link with the rest of the paediatricians in this work: 1. As a practicing GERIATRIC ENDOCRINOLOGIST, I was pissed off when I saw local paediatricians administering IV dextrose solution to newborn babies once their venous blood sugar levels goes below 2.6 mmoL/L irrespective of whether the MAGIC NUMBER in glycaemia is accompanied with symptoms/signs or not! The question "You guys, what are you treating in those babies if they are evidently ASYMPTOMATIC?" went on without answers from both paediatricians and even their STANDARD references and/or local/international guidelines. 2. Many (if not all) child health specialists seem to be motivated by ADVERSE EFFECTS of not acting promptly. Paediatrics books and guidelines worldwide on this topic seems to be forever absorbed by the UNFOUNDED effect of dealing with the matter at hand. 3. As an ENDOCRINOLOGIST, I do consider ASYMPTOMATIC neonatal hypoglycaemia as a TRANSIENT PHYSIOLOGICAL ADAPTATION to EXTRA-UTERINE life until proven otherwise!!! Yes, I know no. 3 above will drive paediatricians in practice mad. However, for those of us in ENDOCRINOLOGY, we treat/intervene pathologies, not ALTERED PHYSIOLOGICAL processes!!! Kindly consider treating HYPOGLYCAEMIA only when there is EVIDENCE (symptoms and signs!) of ill-health in humans. And yes, you may confidently forget what books/local guidelines tell you. You cannot offer DEXTROSE solution to 'almost 50% of your ward' (our case was around 50%), simply because ASYMPTOMATIC babies have evidence of blood sugar level < 2.6 mmol/L. 30% of Pediatrician Dr. Ross Tapangan's patients receive a tympanogram every day in San Benito, Texas Recognizing that ears can be difficult, he performs a tympanogram on any patient with otalgia, hearing loss, or fever to rule out the presence of middle ear fluid or purulence. This narrows his differential significantly before even seeing the patient (an MA or RN can perform the test and it takes less than a minute) and his patients tolerate it very well. He avoids misdiagnosing AOM all of the time! His practice sees about 100 patients/day and tympanometry has helped significantly drive down unnecessary antibiotic use, improved parent's satisfaction, and has been a source of revenue for his practice. The average reimbursement for each patient is between $15- $17. Making this standard of care makes him and his partners significantly better diagnosticians and excellent antibiotic stewards. After 3 months of use, the tympanometer pays for itself - and becomes a nice source of extra revenue for his practice. The AAP/AAO guidelines and the Journal of Pediatric Infectious Disease all recommend tympanometry to avoid misdiagnosing AOM, OME, or AOM in a crying child.
