Reema is a program out of Minneapolis, MN that uses AI technology and community guides to bridge the gap between social determinants of health and health care. Each individual's struggles and needs are different, so every approach is personalized. The program offers care that addresses food insecurity, transportation, housing, social isolation and loneliness, and management of chronic conditions. The result is 29% fewer hospitalizations, more utilization of preventative healthcare measures, and better medication adherence. This is the future of healthcare. Using technology and humans on the ground to address what individuals in our communities need and bridging any identified gaps so that they can live healthy lives. You have a new budget to tackle transportation as a #socialdeterminantofhealth, but the next steps feel a little less defined and you’re not sure how to maximize the value for your patients. Rather creating a generic program, try something a little more patient-centric: Do we know which patients need it most? While transportation is a common barrier to care, it’s rare that a health system knows exactly who would benefit most from a ride. Why? Because it’s normally community health needs assessments or anecdotes that surface transportation as a key need. This year, CMS reporting will be required for inpatient SDoH screenings. Start here and build the process before exhausting your budget.
