🔬 Recent studies highlight gender disparity in Chronic Kidney Disease (CKD) progression; while women have a higher chance of developing CKD, it has been observed that men tend to reach kidney failure at a faster rate. But why does this happen? What are the reasons behind this phenomenon? 🚺 Women are more susceptible to developing CKD due to factors such as a higher incidence of urinary tract infections (UTIs), which can lead to irreversible kidney damage. Additionally, complications during pregnancy, such as high blood pressure or eclampsia, can contribute to kidney damage. 🚹 On the other hand, men are less prone to developing CKD but face a higher risk of reaching kidney failure sooner. This can be attributed to hormonal differences, with higher testosterone levels potentially causing a loss in kidney function. Furthermore, men's kidneys are not protected by estrogen, which can accelerate the progression of CKD. 📊 Analyzing the PointClickCare population, our data reveals a significant increase in female residents diagnosed with CKD. A focus on female reproductive health. The ketogenic diet has demonstrated beneficial effects on metabolic and hormonal parameters that influence reproductive health. Insulin resistance, obesity, and overweight are associated with fertility challenges that can negatively impact the hypothalamic-pituitary-gonadal axis and reduce egg and endometrial quality. In this study, Nicolosi et al. (2024)—a prospective observational case series on the effects of a very low-calorie ketogenic diet (VLCKD) in overweight and obese infertile women—it was observed that 'the compliance level with VLCKD was surprisingly high. Participants experienced weight loss and pregnancy rates improved' (https://lnkd.in/dBP_cDKe). In individuals with PCOS, in a recent systematic review and meta-analysis from Eshaghhosseiny et al. (https://lnkd.in/dUQiXe2g), ten studies including 408 women were analysed. Findings showed that KD significantly decreased triglyceride levels, total cholesterol, and low-density lipoprotein cholesterol (LDL) compared to the control groups. KD also led to a notable reduction in fasting glucose and HOMA-IR . Also, this diet led to a significant decrease in luteinizing hormone (LH) levels and total testosterone levels, while follicle-stimulating hormone (FSH) increased. In another systematic review and meta-analysis from Khalid et al., 2023 (https://lnkd.in/eKWevEca), after 45 days of a ketogenic diet, women with PCOS significantly improved reproductive hormone levels. Findings included a reduced LH/FSH ratio, reduced serum free testosterone, and increased serum sex hormone-binding globulin. Significant weight loss was observed in all the included studies. The importance of CONTEXT when interpreting medical investigations (and why direct-to-consumer testing is not always a good idea). AMH (anti-Müllerian hormone test) has been touted as the fertility 'litmus test' and (possibly) something that will determine your menopause timeline. It is supposed to reflect your ovarian reserve - and has set off many biological clock alarms! But... 🚩 Its inter-cycle variability is 28% 🚩 With that kind of variability, predicting anything for an INDIVIDUAL patient would be very difficult 🚩 What about younger women with low AMH? Younger women with low ovarian reserve have good chances of high-quality embryos and successful clinical pregnancies once eggs are acquired. Surrogate efficacy endpoints are often used to predict patient-relevant clinical outcomes when it would be unethical or impractical to conduct a trial to assess the final patient-relevant outcome. In England, the National Institute for Health and Care Excellence (NICE) is prioritising methods research in surrogate outcomes and collaborating with several international organisations to develop guidance on their use in cost-effectiveness analysis and a joint scientific advice procedure on the proposed use of surrogate outcomes in analysing cost-effectiveness.
