Congratulations to Daniel Goldstein, MD, on the publication of “Textbook of Transplantation and Mechanical Support for End-Stage Heart and Lung Disease.” The first-of-its-kind textbook was co-edited by Dr. Goldstein, Vice Chairman, Cardiothoracic and Vascular Surgery, Director, Mechanical Assistance Program, Co-Director, Center for Advanced Cardiac Therapy, Surgical Director, Cardiac Transplantation, Montefiore Einstein, and Professor, Cardiothoracic and Vascular Surgery, Albert Einstein College of Medicine, and includes chapter contributions from several members of the Montefiore Einstein faculty.    The textbook will provide cardiothoracic surgeons and transplantation specialists from around the world with innovative research covering medical and surgical treatment options for patients with end-stage heart and lung disease. Dr. Goldstein was invited in as senior editor to help define the topics covered in the text, recruit the world’s leading experts as authors, add gravitas to the endeavor, and organize the broad range of content for the project as well as to co-author several chapters. 🔊 We are pleased to share with you the introduction of the Convergent procedure, a hybrid approach to treat therapy-resistant atrial fibrillation. 💓 Atrial fibrillation, especially when it is resistant to traditional therapies, poses significant challenges. The Convergent procedure is a new approach that combines the expertise of both cardiac surgery and electrophysiology to address this condition. By accessing the heart through a small incision, we are able to precisely target and treat the epicardial areas of the left atrium that are known to be often responsible for atrial fibrillation. 🙏 The creation of a new and dedicated hybrid program for the treatment of AFib is only possible with the collaboration of our esteemed colleague, Professor Deisenhofer, from the Department of Cardiovascular Diseases. Scimitar syndrome is an unusual congenital defect characterized by total or partial anomalous venous drainage of the right lung into the inferior vena cava, partial systemic arterial supply, hypoplasia of the affected lung, dextrocardia, and bronchial and pulmonary lobulation abnormalities. Patient with recurrent history of hemoptysis (congenital heart disease surgery): scimitar syndrome with partial anomalous venous drainage of the right lung veins to the inferior vena cava with repair at 8 years of age and tunneling to the left atrium with a Gore patch. A pediatric standard CMR protocol was performed (MR 450w GEM Optima GE HealthCare); in addition, a 4D Flow (processed with Circle Cardiovascular Imaging) was used to analyze the anastomosed area since there was suspicion of occlusion. CONCLUSIONS: Partial anomaly of the venous drainage of the right lung veins, with ectasias of them, as well as the suprahepatic veins. Severe stenosis of the tunneling (of the right pulmonary veins) to the left atrium. Preserved biventricular function. The October issue is available online! https://lnkd.in/g334bF8z On the Cover: Big Changes and Big Gains in US DCD Organ Procurement. Donors after circulatory determination of death (DCD) have historically yielded fewer organs per donor. However, normothermic regional perfusion (NRP), a procurement strategy that entails rapid re-establishment of organ perfusion after determination of death, mitigates the ischemic organ injury incurred after withdrawal of life-sustaining treatment. A center at the forefront of DCD-NRP in the US, the University of Colorado Hospital Transplant Center (Bakhtiyar et al.) analyzed OPTN data and found DCD-NRP organ yields exceeded standard DCD procurement and matched donation after brain death procurement. Using single center data, they report on cost and resource utilization, key considerations along with the need for regulatory guidance and professional infrastructure that our editorialists, Hobeika and Roll, will challenge the rapid expansion of DCD-NRP. Cover design by Megan Llewellyn, CMI, Department of Surgery, Duke University.

A research study co-authored by members of Valley's cardiothoracic surgery and interventional cardiology teams was recently published in Reviews in Cardiovascular Medicine. The authors aimed to describe and compare the characteristics of patients who receive the Sentinel Cerebral Protection System (CPS) - a device developed to reduce the risk of embolic strokes associated with debris produced during TAVR - with those who do not in a “real-world” setting of consecutive transcatheter aortic valve replacement (TAVR) patients, and evaluate its impact on postoperative complications, namely stroke. The study demonstrates that in a “real-world” setting, the Sentinel CPS device is more likely to be used in healthier and less complex patients. In analyses adjusted for illness severity and patient complexity, CPS use did not have a significant effect on the incidence of in-hospital/30-day stroke or the composite endpoint of death, stroke, and bleeding/vascular events at one-year. Breast cancer is the most prevalent cancer in women worldwide, with the standard surgical treatment being lumpectomy, in which the tumour is removed from the breast while sparing as much healthy tissue as possible. However, limitations in surgical navigation often means additional surgeries are required.💉 A team of scientists at Queen’s University have developed a state-of-the-art surgical navigation system using ultrasound, electromagnetic tracking, and proprietary software to improve spatial awareness in lumpectomy surgery.
