Initially, the DCS has been described in severe liver trauma associated with coagulopathy. Over time, Due to the observed advantages, the DSC approach has become standard practice for abdominal trauma with the extent to extra-abdominal trauma and acute abdominal emergencies. Currently, despite the widespread use by the surgical communities, the indications of DCS strategies have not been clearly defined in the absence of high data level and concerns have hence risen about the overuse and related adverse outcomes of DCS approach in surgical trauma patients. However, Advanced progress in trauma-resuscitation techniques have significantly improved the outcomes and reduced the requirement of DSC strategies in trauma injured patients. In sum, DCS remains an important strategy to surgically manage a specific cohort of patients. The incidence of 𝗴𝗲𝗿𝗶𝗮𝘁𝗿𝗶𝗰 𝗮𝗰𝗲𝘁𝗮𝗯𝘂𝗹𝗮𝗿 𝗳𝗿𝗮𝗰𝘁𝘂𝗿𝗲𝘀 has increased in the last decades. Patient-specific factors such as the complexity of the fracture, comorbidities, bone quality and mobility are becoming increasingly important. This is illustrated with the ortho-geriatric case report by Bernd Schmitz, Chief Physician at the Clinic for Orthopaedics, Joint Replacement and Trauma Surgery at Elisabeth-Hospital Thuine, Niels-Stensen Clinics. A 85-year-old female patient had sustained a complex right-sided acetabular fracture with central dislocation of the hip joint. She was treated with the 𝗺𝗼𝗱𝘂𝗹𝗮𝗿 𝗿𝗲𝘃𝗶𝘀𝗶𝗼𝗻 𝗰𝘂𝗽 𝗠𝗥𝗦-𝗧𝗜𝗧𝗔𝗡 𝗠𝗮𝘅𝗶𝗺𝘂𝗺 (with strap, screw fixation and iliac peg 70 mm) using the minimally invasive DAA with expansion acc. to Levine. The revision cup ensures 𝗵𝗶𝗴𝗵 𝗽𝗿𝗶𝗺𝗮𝗿𝘆 𝘀𝘁𝗮𝗯𝗶𝗹𝗶𝘁𝘆 even in complex defect situations. The 𝗶𝗻𝘁𝗿𝗮𝗺𝗲𝗱𝘂𝗹𝗹𝗮𝗿𝘆 𝗽𝗲𝗴 𝗮𝗻𝗰𝗵𝗼𝗿𝗮𝗴𝗲 in the iliac bone enables correct and secure positioning of the cup. A 𝘀𝘁𝗮𝗯𝗹𝗲 𝗳𝗶𝘅𝗮𝘁𝗶𝗼𝗻 was achieved, which led to a 𝗴𝗼𝗼𝗱 𝗳𝘂𝗻𝗰𝘁𝗶𝗼𝗻𝗮𝗹, 𝗽𝗮𝗶𝗻-𝗳𝗿𝗲𝗲 𝗿𝗲𝘀𝘂𝗹𝘁. The patient was mobilised with pain-adapted full weight bearings of the operated limb on a high walking frame under physiotherapeutic guidance. By regaining everyday autonomy as early as possible, geriatric patients can avoid a longer period of post-inpatient care dependency. Arterial septal defect🫀⬇️⬇️ 🎬Here's an image to keep on hand to remember the location of common ASDs . . . 🗣Secundum ASDs are the most commonly encountered atrial septal defects we see. They occur in the "secundum" portion of the atrial septum (near the center of the fossa ovales). We generally close most secundum ASDs, however we primarily look for aspects of right heart enlargement (without significant pulmonary HTN) before closure. Closure can be done either catheter based or by surgery. Primum ASDs are associated with Down's syndrome and possible complete AV canal defects. Sinus venosus have association with anomalous pulm vein return and unroofed coronary sinus are associated with persistent left SVC! 5 ASDs you should know!
