A patient who shares their positive experience in a story is recommending you in a way that is memorable and full of opportunity for your organization. Luigi Pirandello, the Italian playwright said: “A fact is like a sack which won’t stand up when it is empty. In order that it may stand up, one has to put into it the reason and sentiment that caused it to exist.” A story can efficiently convey multiple reasons why someone should choose you. Greenwich Hospital in CT created a video to celebrate the opening of a new wing. A patient told her story of how newly obtained equipment for ambulance crews helped her survive a heart attack and how cardiac rehabilitation restored her vitality. Her results included: “I’ve seen four more grandchildren come into the world. I feel great!” Hospital executive Jeff Atwood invited managers to bring employees who had been recognized by patients or family to hospital board meetings. The manager would tell the board how meaningful the associate’s kindness was for the patient or family. This process : - created memorable experiences for the staff member, the manager and the board.   - reinforced the organization’s mission, vision and values.  - provided good stories to share in the community. Happy Leap Year! As Feb. 29th only comes once every four years, it serves as an excellent reminder to review and update your advance directive, also known as a living will. A living will 📄 is a legal document outlining your preferences for medical care if you become unable to communicate or make decisions for yourself due to serious illness, injury, or incapacitation. It specifically addresses end-of-life care decisions, such as the use of life-support machines, resuscitation efforts, and other medical interventions. By having a living will, you ensure that your healthcare providers and loved ones understand your wishes 🌟, providing clarity and easing the burden of making difficult decisions during emotional times. Creating a living will requires thoughtful consideration and discussions with your healthcare provider, family members, and possibly a legal advisor. It's a way to maintain control over your healthcare, ensuring that your values and preferences are respected even when you can't voice them yourself. Importantly, it can be updated or revoked at any time, allowing flexibility as your health status or views change. Remember, a living will is more than just a document; it's a compassionate guide for your loved ones and healthcare team 🩺💕, ensuring that your wishes are honored. After completing your living will, ensure your physicians and key family members, close friends, and/or religious leaders have a copy. Bring a copy of your advance directive to your next CCHS appointment, or drop it off at the front desk, and we will add it to your file. The recently released Support at Home (SaH) program handbook (see the link below) proves to be an interesting read as it provides important insights into the new program. I note the following key points. There is a 'no worse off' principle for current HCP recipients (plus those on the national priority list) transitioning to the new program with funding equivalent to their current HCP level. There will be a single assessment system with participants receiving a support plan and notice of decision that will outline their quarterly budget allocation and any approved short-term supports. Up to $1,000 or 10% of a participants approved quarterly budget can be carried across quarters. Ten percent of their budget will be set aside for care management. I note that participant's care management budget can be pooled by providers to deliver care management to all their participants. The sector will need to model the 10% care management cap to determine how they will manage delivering these important services to participants under the new program. The handbook talks about price caps for services and no separate administrative fee. Adequacy of pricing is paramount. I note participants will be able to self-manage, but the provider will remain responsible for the quality and safety of services delivered. Participants are to pay contributions only on the (non-clinical) services they receive, with contributions calculated at a set percentage of the service price. There is will be a $130,000 lifetime cap on individual contributions. We know there is currently a growing wait list for HCP recipients and the handbook states that there will likely still be a wait time that carries over into the new program. It does note however, that the target wait time to receive a service will be three months from July 2027. Wait time must be addressed prior to then. It does state that if wait times exceed expectations (how will this be defined?) participants will be assigned an interim allocation of their SaH approved funding (set at 60% of their allocated budget). We need to understand how this would work. Initially there will be a single provider model, but with the possibility of a multi-provider environment when CHSP is integrated into the SaH program. Where responsibility will lie in a multi-provider environment is unclear. Providers will be paid retrospectively for services delivered, with Services Australia paying providers for the services (lees any contributions due from the participant). Providers will have to manage collection of any participant contributions. We are now waiting for the release of indicative pricing later this year, before the release of final pricing early in 2025. The adequacy of price recommendations by the independent pricing authority is key to a sustainable sector under SaH. The sector looks forward to more detail to better understand the new program.
