Real-time identification of life-threatening necrotizing soft-tissue infections using indocyanine green fluorescence imaging Significance: Necrotizing soft-tissue infections (NSTIs) are life-threatening infections with a cumulative case fatality rate of 21%. The initial presentation of an NSTI is non-specific, frequently leading to misdiagnosis and delays in care. No current strategies yield an accurate, real-time diagnosis of an NSTI. Aim: A first-in-kind, observational, clinical pilot study tested the hypothesis that measurable fluorescence signal voids occur in NSTI-affected tissues following intravenous administration and imaging of perfusion-based indocyanine green (ICG) fluorescence. This hypothesis is based on the established knowledge that NSTI is associated with local microvascular thrombosis. All NSTI cases exhibited prominent fluorescence signal voids in affected tissues, including tissue features not visible to the naked eye. All cellulitis cases exhibited a hyperemic response with increased fluorescence and no distinct signal voids. Median lesion-to-background tissue SBRs based on snapshot, IS, TTP, and IMAX parameter maps ranged from 3.2 to 9.1, 2.2 to 33.8, 1.0 to 7.5, and 1.5 to 12.7, respectively, for the NSTI patient group. All fluorescence parameters except TTP demonstrated statistically significant differences between NSTI and cellulitis patient groups. Parasagittal and Parafalcine MENINGIOMAS : UPFRONT vs ADJUVANT RADIOSURGERY; a systematic review and meta-analysis Fernando De Nigris Vasconcellos et al., Neurosurg Rev, 2024 Mar https://lnkd.in/di5xMha9 8 studies involving 821 patients with 878 parafalcine and parasagittal (PFPS) grade I meningiomas - LOCAL CONTROL (LC) was achieved in >80% of all cases, while in the subgroup of patients that recieved UPFRONT SRS LC was achieved in 97% of such cases - TOXICITY was reported in 7.3% of the SRS treated PFPS meningiomas - RECURRENCE was observed in 17.1% of cases - FURTHER SURGICAL APPROACH was needed in 9.2% of cases - SYMPTOM IMPROVEMENT was noted in 33.2% of patients Conclusions: SRS is a SAFE and EFFECTIVE treatment for PFPS meningiomas, both as an adjuvant therapy and as an upfront treatment for often smaller tumors. Pancreatic Protocol reconstruction Had done and producet Renal Angiogram - Urography MPR 👉🏼👉🏼 Doctor's Diagnosis ⚫Mild dilated CBD with prominent pancreatic duct & no detected pancreatic focal lesions…For further EUS assessment if clinically indicated… ⚫Diffuse circumferential mural wall thickening of small bowels prominent at terminal ileum with skip lesions and minimal pelvic free fluids…Inflammatory bowel disease (Crohn’s disease) to be considered…. For clinical correlation and further histopathological assess… ⚫Bilateral suprarenal nodules…likely adenomas. ⚫Mild hepatomegaly. ⚫LT renal stones & Few bilateral small cortical renal cysts. ⚫Pattern of cystitis & mild enlarged prostate. 3️⃣ Female 63 status post extended right hemicolectomy with ileo-descending anastomosis of a known case of transverse colon adenocarcinoma suspected leakage. 👉🏼👉🏼 Doctor's Diagnosis ⚫The ileo-descending anastomosis shows no evident surrounding sizeable collections or contrast leakage, with mild dilatation and circumferential mural thickening of the afferent limb of ileal loops reaching up to 7 mm (likely post-operative edema tous changes). ⚫Mild free mainly left peri-renal , posterior para-renal , left para-colic and to a lesser extent peri-splenic, lesser sac and anterior peri-hepatic pneumoperitoneum, likely post- operative sequels. ⚫Mild free abdomino-pelvic ascites. ⚫Bilateral mild pleural effusions ⚫Liver: Normal size. Normal attenuation features. Left hepatic lobe hypodense focal lesions are noted adjacent of the falciform ligament showing no considerable enhancement on either arterial or porto-venous phases suggesting focal areas of fatty infiltration (for follow up). ⚫Another small 5 mm caudate lobe para-caval non- enhancing hypodense lesion, likely representing hepatic cyst , for follow up. ⚫Gallbladder and biliary tree: shows multiple millimetric dense stones intermingled with minimal biliary mud. ⚫Adrenals: a well-defined right supra-renal rounded focal lesions measuring 1.1 cm along maximum diameters, presenting HU of 9 on non-contrast images ( less than 10) denoting lipid rich adenoma. #MSCT #siemenshealthineers #siemens #computedtomography #angiography #surgery #cancer #ICU #radiographer #hospital #radiology

Optic Nerve Head Infiltration in Presumed Tuberculous Uveitis To report the clinical and multimodal imaging findings of presumed tuberculous optic nerve head (ONH) infiltration in 3 patients at a tertiary eye care center. In addition to unilateral presumed tuberculous ONH infiltration, all patients had clinical findings suggestive of tuberculous choroidal involvement which was confirmed by Indocyanine green angiography (ICGA). All patients were treated successfully with a combination of antituberculous regimen and systemic corticosteroids. Previous reports of tuberculous ONH granuloma have been described in patients suffering from miliary TB.Our cases differ from earlier reports in that all had no active pulmonary TB, the anterior segment examination was completely normal, and vitritis was absent. It is noteworthy that the ICG demonstrated that the choroidal involvement was more extensive than what was seen clinically. It is well-known that in some pathological thyroid conditions, the eyes can also be affected. Thyroid eye disease is the most extreme example of eye pathology caused by a pathological thyroid condition. In the current study, we measured the plasma levels of thyroid-stimulating hormone (TSH), free triiodothyronine (fT3), free thyroxine (fT4), thyroglobulin (Tg), thyroglobulin antibodies (TgAb), and thyroid peroxidase antibodies (TPOAb), in addition to 20 ocular parameters in 4633 healthy adults (10 for each eye, including corneal radius, corneal thickness, anterior chamber depth, anterior chamber angle, lens thickness, posterior chamber length, axial length, intraocular lens power (IOL), spherical power, and cylinder power). This is, to the best of our knowledge, the largest study inferring the effect of thyroid activity on ocular parameters. The most important result of this study was that patients with hyperthyroidism had thicker corneas compared to euthyroid individuals. Intraocular lens power was increased in patients with clinical hypothyroidism, while spherical power was increased in euthyroid individuals with positive antibodies.
