Navigating the job market as a final-year resident or fellow can be overwhelming. Beyond location and compensation, you must dive deep to find the right fit. “There are lots of questions to answer: Is there a need for my specialty in the service area? Will I have the resources I need? What is the local payer mix?” says Leah Grant, MBA, President of AMN Healthcare Physician Solutions. “Physicians considering a career change are taking a step that will have profound consequences for themselves and their families,” Grant adds. When I was graduating from residency, I was presented with 4 career options for the next stage: 1. Become a hospitalist (I didn’t want to do that) 2. Do a fellowship (I initially wanted to, but I changed my mind when I did a rotation, and I didn’t like it) 3. Go to outpatient private practice (which they all seemed to look down on, for some reason) 4. “Traditional medicine” (combination of clinics and rounding on your own patients)- not many generalists do that these days. No one told me about: 1. Locums 2. IME work 3. Industry 4. The path to administration 5. Consulting 6. Utilization management 7. Joint Commision work 8. Academia 9. Locums and per diem work 10. Cross specialty work 11. Starting my own private practice 12. Starting a joint practice with other physicians. 13. Telemedicine (although, that was probably in its infancy back then, if it even existed) If you don’t know what all your career options are, how can you truly say what you like and don’t like? As much as I loved my job, I felt stuck. I trained for years and was good at what I did. Yet I was frustrated by the "broken" system. Increased workload, decreased reimbursement. More meetings, more scrutiny, less autonomy. Someone with no medical background looking over my shoulder. The same patients...just different names and faces. No longer "healthcare," but "sick care." Then, one day, at the scrub sink, I asked my colleague, "how long are you going to keep doing this?" He replied "until I die I guess." And, suddenly, I woke up! I didn't want to be in the Operating Room until I died. I had given up so much. Sacrificed so much. Missed so much. It was time for a change. Not everyone will want to do this. And that is perfectly okay. For me, and my World, it finally made sense. I wanted my family back. I wanted my time back. I needed my life back. Over time, in tiny bits of time, I made changes. I worked. I failed. I learned. Now, I don't have a full time job. I am a co-owner of a national Coaching business. Today, I help more people than ever before. Truly, health care. For me, the system felt "broke." And my life felt "broke." And I had to be honest. With myself. Despite my fears. So that is my message....whatever system you're in. Never allow fears to keep you there. I know it's scary. But, you can change. Your life can change. Diversify your medical career to avoid burnout One of the most rewarding aspects of my career has been the ability to utilize my medical degree in non-traditional ways. This versatility has paradoxically reinforced my dedication to primary care. Many physicians face demanding schedules, seeing patients for up to 60 hours a week and managing around 30 patients a day. These are the ones who often experience burnout. Through my discussions with thousands of physicians and my extensive reading on the subject, I've discovered that those who use their medical degrees to pursue their passions and diversify their activities tend to avoid burnout.
