As we wrap up Older Americans Month, Anthony Perry, MD, our Chief Clinical Officer and Geriatrician, discusses our inclusive care model. This model includes a dedicated care team of nurse practitioners, social workers, care navigators, and physicians, all working together to provide a seamless healthcare experience rooted in trust. With increasing population complexity, understanding patients from multiple dimensions is essential. A reflection of the Patient Experience Symposium - The session led by Tommy Whitelaw, the National Lead of Person-Centered Voices at The Alliance of Scotland, was nothing short of inspiring. Tommy's personal experience as a family caregiver brought a powerful and heartfelt perspective to the critical role of compassion in our work with clients, patients, care, and families. The session emphasized the importance of using a person-centered framework that revolves around key principles like “What Matters to You?”, Intelligent Kindness, and active listening. One of the standout messages was the need for us all to pause, reflect, and consider the impact we have—not only on the people we serve but also on our colleagues and ourselves. Tommy also reminded us of the importance of self-care as a foundation for being able to truly care for others, urging us to cultivate more compassionate workplaces and systems. Let’s keep this vital conversation going and work together to create environments where kindness, listening, and understanding take the lead. #PersonCentredCare #CompassionInAction #WhatMattersToYou #IntelligentKindness #SelfCare #ActiveListening #Caregivers #WorkplaceWellbeing #PatientExperience At the Symposium, ImageThink visualized key insights and discussions.

There's a lot wrong with primary care for older adults today. Our CEO Jack Stoddard joined Sonya Larson on the latest episode of Medicarians Live to share how Patina primary care is flipping the paradigm on primary care for adults 65 and older. Because according to Jack, "when you take the time to build the model our loved ones should have, it drives the results they deserve - a better experience and better quality at a lower cost." 🌈❤️ 𝐉𝐮𝐥𝐲 𝐬𝐩𝐨𝐭𝐥𝐢𝐠𝐡𝐭: 𝐄𝐭𝐡𝐢𝐜𝐚𝐥 𝐝𝐞𝐜𝐢𝐬𝐢𝐨𝐧-𝐦𝐚𝐤𝐢𝐧𝐠 𝐚𝐧𝐝 𝐩𝐚𝐥𝐥𝐢𝐚𝐭𝐢𝐯𝐞 𝐜𝐚𝐫𝐞! As we look back on our focus for July, it’s clear that ethical decision-making and palliative care are critical components in neonatal health. Here are some key takeaways: 📌 𝐓𝐫𝐚𝐧𝐬𝐩𝐚𝐫𝐞𝐧𝐭 𝐜𝐨𝐦𝐦𝐮𝐧𝐢𝐜𝐚𝐭𝐢𝐨𝐧: Ensuring parents are integral to the decision-making process through clear, honest, and compassionate communication. This builds trust and supports informed choices. 📌 𝐅𝐚𝐦𝐢𝐥𝐲-𝐨𝐫𝐢𝐞𝐧𝐭𝐞𝐝 𝐩𝐚𝐥𝐥𝐢𝐚𝐭𝐢𝐯𝐞 𝐜𝐚𝐫𝐞: Prioritising the quality of life for infants with life-limiting conditions and their families. Sharing all relevant medical information, prognoses, and care options while respecting family values and rights. 📌 𝐎𝐯𝐞𝐫𝐜𝐨𝐦𝐢𝐧𝐠 𝐛𝐚𝐫𝐫𝐢𝐞𝐫𝐬: Addressing language, educational, cultural, and socio-economic barriers to ensure effective communication and understanding. Healthcare professionals must strive for clarity and compassion in all interactions. By focusing on these essential aspects, we can improve the support and outcomes for preterm and ill newborns and their families.
