On Improving Patient safety The most common approach to improving safety is learning from errors and near misses. Is this good? Yes! Is this enough? Of course not! Although this approach led to significant improvements in patient safety, the learning and practice change does not happen until *AFTER* the undesired event. Another approach that is used to improve patient safety is failure mode and effects analysis (FMEA). FMEA is a proactive technique that identifies potential failures, their probability of occurrence, the severity of harm they might cause and the odds of it to escape detection. All these numbers are added to a scoring system and then used to identify events that have highest risk, highest likelihood and/or are most likely to escape detection. I personally consider FMEA as an essential tool in risk management and an approach that can be used in quality improvement initiatives #RiskManagement #QualityAssurance #QualityImprovement #Mitigation #FMEA #PatientSafety #ContinuousImprovement

A couple of weeks ago Julian Taylor, P. Langdon Dement, MS, CSP, AEP and I had a conversation about how to deal with #regulators when they turn up at your business. Hopefully there is something useful there for you. Here are some of my tips: 1. Be prepared - make sure your workforce (especially front line) people know who to call and what it is appropriate to discuss 2. Verify they are who they say they are, especially if they are unannounced (you'll find out why later in the podcast) 3. Make sure your visitors are escorted at all times (log arrival and departure times, retain these) 4. Whoever escorts them should be their single point of contact and consider if there should be a second person (for corroboration or to cover for when the phone rings, someone speaks to you, etc.), you don't want them to be on their own as you remain responsible for their safety on your site 5.
