As we navigate the complexities of health financing for Universal Health Coverage (UHC), the concept of procedural fairness emerges as a critical pillar. Drawing from the insightful article from HPP Journal LSHTM by Unni Gopinathan Elina Dale, and David B Evans, we see the importance of fair processes in health financing decisions, especially amid global economic shocks and persistent inequalities. The article introduces a framework focusing on equality, impartiality, and consistency, essential for sustainable health financing. Examples from Thailand, South Africa, Ukraine, The Gambia, and Tanzania highlight the real-world application of procedural fairness. For instance, Thailand’s inclusive approach to health benefit design and South Africa’s legislative process for the Health Promotion Levy are notable successes. However, challenges remain, such as Ukraine's need for respectful opposition engagement and The Gambia's resource limitations. Recommendations: 1. Equitable Participation: Ensuring all stakeholders, including marginalized groups, have a voice. 2. Strengthening Institutions: Building robust institutions to implement fair processes effectively. 3. Balancing Expediency and Inclusion: Managing the tension between rapid decision-making and the need for broad representation. The article rightly points out the difficulties in achieving procedural fairness when powerful stakeholders with self-serving interests dominate. Strategies to mitigate this influence are crucial for genuine inclusiveness. The Accountability for Reasonableness (A4R) framework, initially proposed by Daniels and Sabin, serves as a foundation for the discussion. This framework emphasizes transparency, relevance, appeals, and enforcement as key conditions for fair priority setting. The Interdisciplinary Research Leaders (IRL) Program of the Robert Wood Johnson Foundation, in collaboration with the Journal of Participatory Research Methods (JPRM), is thrilled to announce the publication of a special issue on “How Communities and Research Institutions Work Together to Dismantle Structural Racism and Advance Health Equity.” I serve the IRL as Associate Director of Research, and had the privilege to work as a co-editor for this issue. And I can tell you that JPRM is a phenomenal journal with a visionary mission! The issue contains 15 articles addressing the challenges and successes involved in research prioritizing meaningful community collaboration and shared leadership between those in research and community settings. The articles come from a wide range of U.S. geographies, from California’s Central Valley to rural Missouri to urban Boston, and describe a variety of participatory research methods used to address structural issues such as housing equity or barriers to health care, for an array of communities, including immigrant or minoritized communities, women, children, youth, and disinvested urban and rural communities. The authors include current IRL fellows, IRL alumni, and other community-based researchers focused on conducting community-engaged, action-oriented research to reduce/eliminate structural racism and make progress toward health equity. How can we bridge the gap between academia and policy more effectively? In this recent short report "A modified action framework to develop and evaluate academic-policy engagement interventions" by Mäkelä and colleagues they present the modification to the SPIRIT framework, aimed at enhancing academic-policy engagement across various sectors, including health, social, environmental, and economic policies. 🔍 Key Findings from the report: 👉 There has been a significant increase in theories, models, and frameworks for translating research into practice. However, few focus specifically on engagement between academia and policy. 👉 Challenges in evidence-informed policy-making are gaining global attention. Despite the growing number of academic-policy engagement interventions, there is a notable lack of published evaluations. 👉 This article provides an action framework designed to guide academics in the development evaluation and facilitation academic-policy engagement interventions, ultimately supporting the use of evidence in policy decision-making. 👉 The study illustrates the power of modifying existing useful frameworks rather than creating new ones. 👉 The study emphasises the necessity for sustainable, multidirectional interactions between academics and policymakers.
