The U.S. spends more on health care than almost any other country. But patients increasingly are getting less in return, and enduring long waits to get less face time with their clinician, Axios' Tina Reed writes. Why it matters: Shrinkflation is hitting a sector that accounts for almost one-fifth of the economy — eroding the doctor-patient relationship, and leaving many turning to urgent care clinics or telehealth services. The trend is being felt especially in primary care, but also in many specialties. An Axios-Ipsos survey last month found that nearly 1 in 5 respondents said they had to wait more than two months to see a primary care physician or specialist, with waits trending longest in the Midwest. The most frequently cited specialties with long wait times included neurology (26%); ear, nose, and throat (26%); psychiatry (20%); and OB/GYN (17%). Primary care stood at 19%, per electronic health records company Tebra. 🔭 The big picture: There's been across-the-board growth in patient volumes, largely from pent-up demand for services many put off during the pandemic. At the same time, tens of thousands of doctors have left the field due to burnout and other factors, and surveys indicate even more plan to retire early.

In the Telehealth Success book, there is an entire section on Stigma and Disease. There are a number of reasons that people do not want/do not like going to the physician and it worsens when someone has a disease that is stigmatized. Telemedicine has been shown to decrease. It improves patient outcomes, reduces stigmatization and decreases treatment burden for patients. It allows patients who might be nervous to go in-person for treatment to get access to care. One of these diseases is obesity. We have seen the number of direct to consumer companies prescribing medications, of course. But there are also companies that offer a full experience - from counseling, health coaching, nutrition, exercise and also, if needed, medications. The research on outcomes also pans out. In an observational study over 6 months, using video or video plus in-person visits had clinically significant weight loss. It is interesting that the study compared hybrid vs full telemedicine visits, rather than telemedicine vs in-person. It is a step toward how telehealth is now accepted and the norm. There are other examples of stigma and disease - some of it changes depending on culture. Mental health is a good example of this. While it is less stigmatized in some places, there is still needed forward movement (and investment). With more research, I predict we will see more of these examples. For anyone who feels any stigma, sometimes a virtual visit is a good start. Remote monitoring of kidney function: what matters to patients and healthcare professionals? A guide for medtech, device developers, evaluators and healthcare commissioning and procurement. Published today! One of the hardest aspects of the What Works Centre for Wellbeing's closure has been deciding what to step away from, what to do less of, and what to try to deliver against all odds. This project definitely fell into the last bucket. The sensible thing would have probably been to bail out in Jan/Feb - but if you've worked with me, you'll know I'm not a fan of the sensible option, when there is a better one just in reach. Many of you may rightly question my assessment of 'just in reach.' I am indebted to so many people who helped make this happen. Libby Keck and Anindita Gosh at Q Lab & The Health Foundation who were the best funding partner I've worked with; understanding and encouraging in equal measure.
