Development & validation of novel #MCS risk stratification tool for mortality for children supported with ventricular assist device: the Advanced Cardiac Therapies Improving Outcomes Network (ACTION) VAD score, including 4 components: 🫁 ventilation 🩸 advanced organ support: dialysis or #ECMO 🩺 diagnosis ⚖️ size (weight ≤5 kg) Patients classified into 4 classes: class 0 🟢 (4% mortality at 1 year), class 1 🟡 (16%), class 2 🟠 (21%), class 3 or higher 🔴 (42%). Cardiogenic pulmonary edema and atelectasis may exacerbate RV dysfunction in patients on VA #ECMO through increased lung elastance and RV afterload. Invasive MV settings, particularly PEEP, can help to improve RV performance optimizing lung recruitment + minimizing alveolar overdistension. Here report of in vivo RV pressure-volume loops measured during decremental PEEP trial, leading to identification of optimum level from a cardio-respiratory viewpoint. 🚨 **Advancements in Ischemic Stroke Management** 🚨 The management of ischemic stroke has significantly evolved, thanks in part to the findings of the ECASS trials. These trials extended the therapeutic window for administering tissue plasminogen activator (tPA) to up to 4.5 hours after symptom onset, confirming the earlier results of the NINDS study. As a result, the American Heart Association (AHA) and American Stroke Association (ASA) now recommend tPA for eligible patients presenting within this 4.5-hour window, offering hope and improved outcomes for many.
