In our last week's discussion on #PatientSafety in MP Shah, one of the questions that came up was, 'how do you ensure a culture of safety?' Two tips from me: 1. Always blame the system, not the person, for any errors. Given then same systemic circumstances and conditions, another person would suffer the same outcome. 2. Separate any disciplinary/accountability matter from the Root Cause Analysis exercise. My fellow panelist shared a tip whereby his institution encourages and rewards the department with the most reported incidents. Talk of celebrating failures to breed success. We need to empower our staff on self reporting of quality incidents
