We are excited to share the latest peer reviewed publication on Restrata, titled “Utility of synthetic electrospun fiber matrix in general surgery: From head to toe” is now published online at Surgery. Authored by Dr. Susan Hagen, this retrospective study concluded that “the synthetic nature of the matrix limits the risk of inflammatory response and is well tolerated, which demonstrates initial proof of concept of synthetic electrospun fiber matrix treatment in a variety of complex wounds. The shortage of anesthesia providers presents a serious problem for ophthalmology, as the number is expected to decrease by 30% over the next 10 years. In this Ophthalmology Times article, Marc Bosem, MD shares how office-based surgery is poised to address to the anesthesia shortage. “Most OBS cases are performed with light oral sedation over IV sedation. Patients are more relaxed, and there’s less risk of surgical complications with class A sedation. This procedure was performed 6 days ago in my Clinic surgery, in Balneário Camboriú, Santa Catarina, Brazil on a 65-year-old patient, with urinary retention using an indwelling bladder catheter for 20 days, wishing to reestablish spontaneous urination and preservation of ejaculation. After presenting all possible surgical techniques, the patient chose to undergo MIST - Urolift System, in which 7 implants were applied to the lateral lobes of the prostate., Procedure performed with sedation, duration of 15 minutes and outpatient discharge without delayed bladder catheter after 24 hours. When I think about advancements in technology I think about how we are now better able to address patients quality of vision and thus quality of life in situations where we might have historically not felt comfortable addressing. Heres a video of a patient who although was 20/20 post cataract surgery, was suffering from clinically significant vitreous opacities and had a really hard time with daily tasks due to these opacities. He also had mild pigmentary glaucoma and despite SLT in the past, was on 4 topical IOP lowering meds and oral CAI not able to stay compliant with the regimen. We therefore decided to perform an in office (OBS) 1 Step Limited Vitreous Removal procedure and KDB goniotomy using topical and oral anesthesia. This patient is already 1 month post op and now is just on a combination topical IOP reducing drop and not having issues with vitreous floaters. This is a patient I would have, in the past, told, "your pressures are doing well and the floaters are normal, so we will see you back in 4 months." Not appreciating the patient's daily life issues were not actually being addressed. The meta-analysis conducted by Authors delved into various SMAS facelift techniques, yet failed to reveal any statistically significant differences between them. Instead, the pivotal factor in preventing hematoma during facelift surgery lies in the surgeon's meticulous control of hemostasis intraoperatively. This article also suggests additional measures, including careful blood pressure management, strategic use of quilting sutures in high-risk patients, mitigation of postoperative nausea and vomiting (PONV), utilization of local anesthesia, and contemplation of tissue sealants. When employed collectively, these measures are likely to have an additive effect in reducing hematoma rates post-facelift. However, it's crucial to note that appropriate patient selection remains paramount. Nonetheless, it's worth mentioning that some referenced manuscripts have limited sample sizes, highlighting the need for further research to strengthen these findings.
