Small Step or Giant Leap? Expanding the Acute Stroke Thrombolysis Window to 24 Hours Author: Vivien H. Lee, M.D. Author Info & Affiliations Published June 14, 2024 N Engl J Med 2024; 391:273-275 DOI: 10.1056/NEJMe2405846 VOL. 391 NO. 3 In geographic regions that lack resources to develop thrombectomy programs, the administration of tenecteplase in the 24-hour window after stroke onset may be a practical treatment alternative that could improve functional outcomes in patients with large-vessel occlusion on an international scale.

With severe ARDS stages; the patient‘s lung is not recruitable any more . In this case: PEEP of < 10 cmH2O together with prone positioning is the desirable choice.
Respiratory Therapist and Medical Simulation Specialist | Educate clinicians how to deal with mechanical ventilators and give them solutions for medical training.
Lung recruitability assessment is so critical. Depending on CT scan guidance, there're many ARDS-related factors we should think about when it comes to the context of lung recruitabilty Is it - Focal or nonfocal? - Moderate or Severe? - Early or late? - Pulmonary or nonpulmonary? As many studies showed that nonpulmonary, nonfocal, severe, early stage ARDS most probably recruitment maneuver would be beneficial for them.

Case of the week: how would you treat? We admitted a 63-year-old patient with NSTEMI and posterior wall ischemia. We found an atypical LCA origin from the right valsalava sinus and an occlusion of the RCA. After multiple dilatations of the RCA, there was a large flow-limiting thrombus proximal to the RCA and an occlusion of the RPLD, see my next post. What is your strategy in these cases? I look forward to your comments!

Our cohort study of three tranexamic acid dosing regimens in major traumatic haemorrhage showed no significant differences in clinical outcomes. This study provides evidence to support the use of two, pragmatic, alternative dosing strategies to that used in the CRASH-II trail @FinnGunn @AteeqAlmuwallad @Andrea91Ross @Paulvulliamy https://lnkd.in/eJZpEGQ9

Management of Acute mesentric ischemia
Unless it’s in your differential, you’re going to miss mesenteric ischemia. This is scary stuff which I describe to patients (but mostly families) as a heart attack, but for the intestines. This is a great algorithm on managing it. 🎩 tip to the authors. https://lnkd.in/d9dq5fWW

The manifestation of an immune response can result in #ocular inflammation impacting study duration, power, and the ability to meet objectives. Using immunosuppression can help mitigate inflammatory impact but may not completely quench the response. In this presentation, we explore the ethical and scientific considerations for using immunosuppression in non-clinical ocular studies and the benefits, concerns, and strategies for its use. https://okt.to/RSk7fF
