New Case Study: Confirmation bias The background of this incident involves a pilot mistaking Cambridge Airport for Duxford Airfield. 1. Situation The pilot was flying VFR in a biplane with limited avionics, intending to land at Duxford Airfield (EGSU). Having never flown to Duxford before, the pilot sought tips from a friend familiar with the airfield. The friend described Duxford as having a south-westerly hard runway with a parallel grass runway and a petrol station on the final approach path, with hangars and a tower to the north on the right-hand side during the approach. 2. Task The task was to navigate to and land at Duxford Airfield safely, using the provided visual cues and following VFR procedures. 3. Action The flight was uneventful, and the pilot contacted Duxford Information to announce a straight-in approach to runway 24. The Duxford FISO confirmed and later instructed the pilot to land at their discretion. On final approach, the pilot passed a petrol station, saw hangars on the right, and landed on the hard runway with a parallel grass runway, believing everything had gone as planned. However, they had mistakenly landed at Cambridge Airport (EGSC), which shares similar characteristics with Duxford but is 8 miles away. 4. Result Despite landing at the wrong airport without clearance, there were no harmful outcomes. This incident underscores the importance of confirming locations to avoid confirmation bias. Wrong airport landings have a long history, but modern avionics can help reduce their frequency. Nonetheless, even with GPS, pilots can misidentify airfields and convince themselves they are at the correct destination. The lesson learnt is to thoroughly verify the intended location, utilise all available navigation aids, and remain cautious of confirmation bias to ensure accurate identification of the destination airfield. #aviation #privatepilot #pilot #trending #pilotlife

CAAC Updated CCAR Part 129 (CCAR-129) (Part 2) 6. Initial Application for Operation Specifications After obtaining the regular route operation license from the CAAC, the foreign air carrier (hereinafter referred to as the applicant) can apply for the CCAR-129 operation specifications. The initial application is divided into the following four phases: 6.1 Application 6.1.1 Offline consultation The applicant can consult with the regional administration bureau to understand the applicable requirements and the application materials to be submitted. The management principles and contact information of each regional administration bureau are published on the FSOP (Flight Standards Oversight Program) 129 subsystem. At the same time, the regional administration bureau also needs to understand the relevant information of the applicant, including the fleet size and structure, base distribution, safety management system, and international operational experience. If the applicant is the first foreign air carrier in its home country to apply for the CCAR-129 operation specifications, the regional administration bureau also needs to understand the relevant situation in the applicant's home country, including the applicable aviation regulations, the form of operating licenses, the responsible department, and the contact person. The regional administration bureaus should inform the applicant of the relevant policies and the principles for access to the operational evaluation. After self-assessment, the applicant should submit the formal application through the FSOP system according to the process. The applicant should designate a responsible contact person and provide the contact information during this stage to receive relevant notifications. 🛬 'TENERIFE CALLING ON LINE 1' ? I usually post about aviation events in which communication breakdown is a factor. This event seems different to me. ⚫ On May 6, 2024 American Airlines flight AA2473 was cleared to take off from Orlando International Airport in Florida, USA. ⚫ Moments later, the same ATCO cleared Frontier Airlines flight F91734 to cross the same runway as it headed toward its gate. ⚫ At a speed of about 60 knots, the AA pilot noticed the runway traffic and rejected takeoff. ⚫ It took several moments for the ATCO to realize what was going on and order (redundantly) that the takeoff be aborted. Some thoughts: 🟧 The ATCO seemed very relaxed about the incident. Is this a sign of his inexperience or experience? 🟧 I understand it’s too easy to ‘kick a man when he’s down’. No doubt the FAA is working hard to improve the current sitiation. Just this week the U.S. House of Representatives approved the FAA Reauthorization Act of 2024. 🟧 Should this incident be added to the box labelled “US Controllers Understaffed and Overworked” or are there other factors to consider? Plane crashes and the Chicago Convention 45 years ago, on October 7th 1979, Swissair Flight 316 crashed into the Ellinikon Airport of Athens, killing 14 of its passengers. The pilot and co-pilot were later charged with criminal negligence after an investigation of the crash, that followed international standards to inspect and report plane crashes that have been agreed almost 100 years ago! In 1927, after several plane crashes, several airlines met in Chicago and agreed on a common process to investigate, report and use this knowledge to prevent, airplane crashes. It was later known as The Chicago Convention and this was Annex 13. This decision to analyze failures of equipment or people, led to air travel being one the safest means of transportation today, reducing incidents like the Swissair 316 to almost zero. In the end, all it took was a hard decision to accept failure, to openly discuss it and use this knowledge to prevent it from happening again in the future.
