Blood Interleukin-18 (IL-18) and IL-18 Binding Protein (IL-18BP) Levels Following Midline Laparotomy: A Prospective Randomized Study of Patients With Benign Disease and Patients With Cancer https://lnkd.in/gimBKFsA “`html Study Title: Blood Interleukin-18 (IL-18) and IL-18 Binding Protein (IL-18BP) Levels Following Midline Laparotomy: A Prospective Randomized Study of Patients With Benign Disease and Patients With Cancer Abstract: Background/Aim: The study aimed to investigate the acute phase immune response (APR) in patients undergoing midline laparotomy (MLa) and its association with blood IL-18 and IL-18BP levels and the numeric rating scale (NRS) pain score. Patients and Methods: Blood levels of seven cytokines and high-sensitivity C-reactive protein (hs-CRP) were measured at three time points in 56 patients with MLa. Patient satisfaction at 24 hours post-MLa was recorded on an 11-point numeric rating scale. Results: IL-18 and IL-18BP blood levels decreased immediately after operation (POP1) and then increased significantly at 24 hours after operation (POP2). Patients with cancer had higher IL-18 values compared to those with benign disease. IL-18BP levels correlated with patient satisfaction at 24 hours post-MLa. Conclusion: The study suggests that the acute phase immune response and quality of life are associated in MLa patients, highlighting the potential value of monitoring IL-18BP levels in post-operative care. The groundbreaking results of the NETTER-2 trial herald a significant stride in the fight against advanced neuroendocrine tumors (NETs), particularly in the digestive system. The efficacy demonstrated by Lu 177-dotatate (Lutathera) in combination with octreotide is undeniably promising, showcasing nearly tripled progression-free survival and unprecedented tumor response rates. Led by Dr. Simron Singh, this pioneering study not only challenges existing treatment paradigms but also addresses the pressing need for effective therapies for high-grade GEP-NETs, a particularly elusive subset. #NatureCommunications: Protective effect of TCR-mediated MAIT cell activation during experimental autoimmune encephalomyelitis To summarize, blocking as well as activating MAIT cells via their TCR in EAE revealed that their tissue repair and protective function outweighed their pathogenic potential. This is likely mediated by AREG as an established suppressor of astrocyte pathogenic function in EAE and can potentially be targeted therapeutically by enhancing their cognate antigen activation. Mucosal-associated invariant T (MAIT) cells express semi-invariant T cell receptors (TCR) for recognizing bacterial and yeast antigens derived from riboflavin metabolites presented on the non-polymorphic MHC class I-related protein 1 (MR1). Neuroinflammation in multiple sclerosis (MS) is likely initiated by autoreactive T cells and perpetuated by infiltration of additional immune cells, but the precise role of MAIT cells in MS pathogenesis remains unknown. Here, we use experimental autoimmune encephalomyelitis (EAE), a mouse model of MS, and find an accumulation of MAIT cells in the inflamed central nervous system (CNS) enriched for MAIT17 (RORγt+) and MAIT1/17 (T-bet+RORγt+) subsets with inflammatory and protective features. Results from transcriptome profiling and Nur77GFP reporter mice show that these CNS MAIT cells are activated via cytokines and TCR. Blocking TCR activation with an anti-MR1 antibody exacerbates EAE, whereas enhancing TCR activation with the cognate antigen, 5-(2-oxopropylideneamino)−6-D-ribitylaminouracil, ameliorates EAE severity, potentially via the induction of amphiregulin (AREG). In summary, our findings suggest that TCR-mediated MAIT cell activation is protective in CNS inflammation, likely involving an induction of AREG. https://lnkd.in/eHsXEu8S

📃Scientific paper: Functional Impairment of Endothelial Colony Forming Cells (ECFC) in Patients with Severe Atherosclerotic Cardiovascular Disease (ASCVD) Abstract: International audience; Endothelial dysfunction is a key factor in atherosclerosis. However, the link between endothelial repair and severity of atherosclerotic cardiovascular disease (ASCVD) is unclear. This study investigates the relationship between ASCVD, markers of inflammation, and circulating endothelial progenitor cells, namely hematopoietic cells with paracrine angiogenic activity and endothelial colony forming cells (ECFC). Two hundred and forty-three subjects from the TELARTA study were classified according to the presence of clinical atherosclerotic disease. ASCVD severity was assessed by the number of involved vascular territories. Flow cytometry was used to numerate circulating progenitor cells (PC) expressing CD34 and those co-expressing CD45, CD34, and KDR. Peripheral blood mononuclear cells ex vivo culture methods were used to determine ECFC and Colony Forming Unit- endothelial cells (CFU-EC). The ECFC subpopulation was analyzed for proliferation, senescence, and vasculogenic properties. Plasma levels of IL-6 and VEGF-A were measured using Cytokine Array. Despite an increased number of circulating precursors in ASCVD patients, ASCVD impaired the colony forming capacity and the angiogenic properties of ECFC in a severity-dependent manner. Alteration of ECFC was associated with increased senescent phenotype and IL-6 levels.
