I was honored to speak RISE with Nate Lucena and Katie Martin on the challenges that older adult LGBTQ folks face in navigating healthcare. The combination of the period they grew up in, the still pervasive discrimination in the health system or lack of knowledge, and unique social determinants of health has resulted in a group that gets fewer screenings, has less social support, higher rates of substance abuse, higher rates of being unhoused, and higher rates of certain conditions. A few stats comparing LGBTQ seniors to the general population Only 20% have children, compared to 80% (less support) 2x as likely to be unhoused 39% experience depression compared to 16% 5x as likely to avoid healthcare settings because of fear of discrimination 2x as likely to smoke 2x as likely to live alone 28% hypertensive compared to 21% 10-15% HIV+ compared to 1% 48% have experienced discrimination if they are in senior housing For the transgender community (and this blew me away) Only 38% get mammograms, compared to 89% 26% get pap smears compared to 82% 20% have been denied care 30% have experienced verbal harassment in healthcare settings There are actionable steps you can take as a health plan to ensure that this segment of the older adult population is getting the care that they need...ideally, that starts with collecting the right data (SOGI). Still, another place a plan can start is by providers self-identifying that they are just willing to treat LGBTQ seniors without discrimination. That one step reduces anxiety and is the first step in creating a dialogue with your population. Before we can build cultural competency, let's look at who already has it! The final recommendation is to start within your organization. Your employees will benefit and have a wealth of knowledge. It was exciting to hear about what Baylor Scott & White Health is doing and to read about what SCAN is doing in plan design to support the community. I would also look into what FOLX Health is now doing to support senior LGBTQ populations and Included Health is doing to support employers.
