When I first entered the world of pastoral leadership, it did not enter my mind that part of my role would be calling GPs or dentists to see if they would register my students, but it was. Students coming to school complaining of toothache need to be taken seriously and where children have visible plaque or cavities, this needs to be addressed sensitively with each child and their family. Poor oral hygiene can have a devastating long-term impact on overall health and while we are in the middle of a crisis in dental care, we need to do our best to support families by signposting them to services where possible and advocating for our communities where there is a lack of these services. In Hackney, 62.2% of children did not see a dentist last year, whereas the national figure is 44.6%. The NHS service finder can be used to find out whether there are clinics taking new patients in a town/postcode area. Currently, it shows that for residents of Hackney, there are clinics further afield in Leyton or Stratford possibly still taking new NHS patients. https://lnkd.in/eXXASJcR School staff also need to be aware that some of the behaviours they may be seeing in their classrooms may have this issue at the root.

One of the ways we experience connection with others is through the simple question, “What brings you joy?” Healthcare environments are complex, and it can be difficult to see the opportunities for connection with colleagues, patients, and families. Human connection at the bedside changes the care provided and impacts patient outcomes. A critical care nurse caring for a child and while assessing the child, takes a pause and asks the caregiver at the bedside, “Can you tell me about what brings your child joy? What do they love?” This moment opens a longer, genuine conversation where the caregivers find an opening to share their concerns and fears. This simple question promotes seeing the child and caregivers as unique individuals. Building connections with patients, families and colleagues creates joy. It is the joyful part of life to build connections in our community. At Shifting Frames Consulting, one thing that brings us joy is meeting new people and hearing their stories. We have four projects underway being pilot tested in order for best practices to be promote nationally. These projects specifically empower residents to have control over their personal and community life, and #CNAs to grow in their career and nursing home roles. 👉 We'll be releasing a microsite later this week with "how to" guides. 🌟 Strengthening our voice for consensus of solutions. We're continuing to bring together diverse voices to identify solutions (big and small) we can all agree on. 👉 This week we'll have three consensus-backed federal policy recommendations to move forward with necessary shifts. 🌟 Seeking partners as we can't do it alone. 👉 We need help funding and scaling other promising projects to: incentivize household models, ensure surveyors have training in person-centered care, and make person-centered care planning a part of nursing home electronic health records. Just a reminder to all the clinical providers in Maryland that there is still a huge need for supporting learners in the home setting. We have been struggling with finding providers willing to go in-home and work with clients in the home setting. While I understand that clinic hours are really convenient, the reason we (BCBAs and RBTs) got into this industry is to help the clients that need it the most. That might be a 10 year old after school for 3 hours working on his AAC device learning how to communicate his wants and needs. Or helping an 18 year old learn how to follow a schedule so he can get a job or participate in family responsibilities. This might be helping a 15 year old reduce their SIB to rates that are profoundly life changing for them and their families. Even taking 1 client that is looking for services in the home would take 1 client off a wait list that might otherwise be on it until they age out. One evening client receiving ABA services not only impacts the client, but all of society in a positive way. We can make caregivers happier and healthier, schools safer, taxes lower because less intense support is needed for the long term. We can do better Maryland. We MUST do better Maryland. The stakes are too high for our clients and communities.

As a young healthcare administrator involved in Graduate Medical Education, I remember hearing the Flexner Report discussed as a groundbreaking moment that modernized and improved the training for physicians across the country. No one ever mentioned, or was possibly even aware of, the racist views of the report’s lead author and the impact of the report on the closing of five of seven predominantly Black medical schools - shutting off opportunity for thousands of potential physicians in the future. Another lesson I learned early in my career is that it is impossible to sustainably fix a problem without fully grasping the situation with an open mind, relentlessly asking “why?” and moving beyond superficial answers to get to root causes. Uché Blackstock, MD's recent article tells this story of the Flexner Report and helps us to more fully grasp the situation of the ongoing lack of diverse representation in healthcare. I highly recommend the article and am looking forward to reading her book as well (links below). What lessons can be learned from this story - and what might it teach us about the work we are leading today? 4moThanks for this! I gave up driving about 2 years ago. Fortunately I live now at Residence at Riverbend in Ipswich (assisted living community with about 80 residents). They provide transportation to drug stores, grocery stores (tho we have 3 meals a day…but everyone has a wish special treats). Glad you’re helping seniors this way! Kathy Lique
