Younger adults between the ages of 16 to 64 are very different from older adults and seniors residing in skilled nursing homes. Beside age, their physical, medical, social, psychiatric and substance use issues make them a unique group. Some enter the facility with developmental or intellectual disabilities, trauma-induced hemiplegia and quadriplegia, higher rates and greater severity of psychiatric illnesses and substance-related diagnoses. Others are admitted with chronic neurological conditions like muscular dystrophy, Huntington's disease, Parkinson's disease, ALS, and peripheral vascular disease, to name a few. In 2012, I became a nurse. In 2018, a nurse practitioner. My career includes time in the pediatric intensive care unit, inpatient psychiatry, community mental health, global health, inpatient palliative care, public health, and the emergency department. In speaking about my experiences with non-clinicians, I’ve learned many people assume it is difficult to see how disease can ravage the body. My experience, however, is that the real toll is taken from witnessing the damage from inequitable access and treatment, from violence inflicted on a person by another, and from failures of the health system that result in a preventable loss of health, trust, vitality, hope, and life. In speaking with colleagues, I know I am not alone. This is only one reason why the solution to “burnout” (more accurately described as moral injury) requires meaningful changes to health and social service systems intended to prevent and alleviate human suffering. We need to work together to change the wiring, not replace the bulbs.

Last week, The New York Times reported that although roughly three in 10 adults have been addicted to opioids or have a family member who has been, less than half of those with a substance use disorder have received treatment. Opioid addiction across the nation recorded around 110,000 fatal drug overdoses last year alone, but some proven medications for helping curb drug cravings, such as buprenorphine and methadone, are still not getting to those who need them.
