A recent study from the University of Minnesota Rural Health Research Center found that gaps in access to healthcare can exacerbate risks to the well being of older #rural residents caused by social isolation, injuries, or food insecurity. As the U.S. population ages, many Americans age 65 or older are making their homes in rural communities, and the need for integrated – and sustainable – healthcare solutions has become even more critical. It’s #NationalHealthCenterWeek, and America’s more than 1,400 community health centers are a vital part of the clinical care foundation for neighborhoods and communities across urban and rural America. Access to care is one crucial component of what is needed to solve the challenges of improving health status, especially of rural communities. Federally qualified community health centers provide critical and affordable access to health care for rural residents affected by hospital closures and not yet fully served by remote clinical care or hub and spoke models. Rural populations continue to face significant health care disparities including access to specialty and obstetrical care, higher poverty rates, lower educational attainment, and overall worsening health status compared to urban populations. The Association of American Medical Colleges (AAMC) Research and Action Institute emphasized that we must better define and address how to improve rural health with targeted solutions – including increasing geographic access and reducing barriers to care, as well as expanding social investments in rural communities such as poverty reduction and educational opportunities. A recent study published by the CDC expands on a barrier to care that impacts many patients in the U.S. Nearly 6% of U.S. adults lacked reliable transportation for daily living in 2022—a situation that can have negative health impacts for those unable to get to medical care. At DaVita, patients are surrounded by a care team that works to address social determinants of health, like transportation challenges, in order to meet their holistic care needs. As we continue to expand access to value based kidney care, or care that connects a patient’s kidney needs to their other health conditions, we’re thinking about how we can address a patient’s comprehensive health issues and the circumstances that contribute to their unique needs. https://lnkd.in/gh4kBYXE

“They [CHWs] are better able to understand language barriers, address health literacy needs, confront social isolation, connect residents with housing and food resources, and help with transportation or childcare needs that may be barriers to accessing health care.” Now this is an excellent example of using great communication strategy to ensure/reduce information sharing gaps. Recognizing that there are different groups of people and learning how to effectively spread awareness and gain a more realistic insight/feedback simultaneously.
