The Juniper Community Care Hub is an innovative way to help older adults and family caregivers in Minnesota receive the services that they need by providing an influx of resources and other support. Join us next Thursday as we share our journey on how we adapted our Weatherization program to provide home-health related California Advancing & Innovating Medi-Cal (CalAIM) Community Supports through our partner, CalOptima Health. Webinar Description: Community Action Partnership of Orange County (CAP OC) has leveraged their Weatherization program with Medicaid programs to provide supplemental home modification services to meet the growing needs of underserved clients with mobility issues or asthma. Additionally, they have taken advantage of Weatherization readiness funds to abate asbestos and other deferral issues. By integrating the Weatherization process with non-energy funds, they have created a healthier home environment for participants, improving comfort, health, safety and accessibility. This innovative approach also leverages contract funds from their county Medicaid provider, CalOptima Health for additional community support services. Representatives from CAP OC will describe how this program was developed, highlight key milestones, and discuss current challenges. Presenters will explain how to create similar Medicaid initiatives in other states, and there will be dedicated time for audience questions.

Tech-enabled solutions can help address the health-related social needs (HRSN) of Medicaid enrollees, including, by supporting data collection and sharing, helping health care and social service organizations coordinate care, and providing supports, such as food delivery and transportation services. This brief explores how tech‑enabled solutions can help states achieve their goals to address HRSN and offers examples of how Medicaid agencies can incentivize the use of tech solutions to support HRSN interventions. This working group advances solutions to address the shortage of maternity care providers and facilities, issues tied to health care insurance coverage and affordability, provider bias and access to culturally congruent care, and overall gaps in quality of health care.
